Extra-curricular activities are an important part of your child’s education. We recognize learning
occurs both inside and outside of the classroom. It is the goal of the Carlisle Area School District to
provide a safe, secure learning environment for all its students. In order to provide this type of
environment, we ask that all guests who wish to attend one of our extra-curricular activities, meet the
following requirements.
1. Guests are defined as people who do not attend Carlisle High School, Carlisle Virtual
Academy (CVA), or Carlisle Evening Alternative Program (CEAP).
2. Guests must be at least a sophomore in high school but not over 20 years of age.
3. Guests are limited to ONE (1) per CHS student.
4. Guests must be recommended by an administrator from his/her school.
5. Guests must not presently be suspended or expelled from any school.
6. Guests have no serious behavior problem at his/her present school.
7. Guests will be subject to a random Breathalyzer test.
8. Guests must provide proper identification prior to admission.
9. Guests will not be admitted without this signed and completed form on file.

his/her guest. I am aware of the responsibilities and expectations for participation in such an event
and support the district’s efforts to secure and maintain a safe and enjoyable experience.
