Statement of Commitment to Comply with
OSH Requirements
I, by accomplishing this Construction Safety and Health
Program do hereby commit and bind ourselves to
strictly comply with the requirements of the
Department Order No. 13, series o f 1998
–
Guidelines Governing Occupational Safety and Health
in the Construction Industry and the applicable
provisions of the Occupational Safety and Health
Standards (OSHS).
I (also) acknowledge our obligation and responsibilities to
provide through out the course of the project the
appropriate and humane working conditions and provision
of Personal Protective Equipment (PPE) as added
protection for our workers and employees.
I also (hereby) commit to implement the necessary job
safety and health instructions and training to all our
workers for the entire duration of the project, as well
as ensure the safety of the general public. Authorized
