Covering letter about my clinical audit project
I just moved to Scotland 2 months and 10 days ago.
It’s my first NHS post. I am working now under
supervision of Dr Thomas N. Martin on a project
assessing the accuracy of our aortic stenosis grading by
echo at the department.
We are done with identifying the problem, checking the
standards and data gathering. We are done with the first
cycle, and kindly find below our audit proposal and the
contacts of Dr Martin (Consultant of cardiology) for
reference.
Kind regards,
Bishoy Bishay

Are we over-estimating aortic valve stenosis severity
Is this a re-audit
 YES
 No
Why are you proposing to conduct this audit?
To assess are we over/under estimating aortic valve stenosis severity by echo
comparing the two methods of quantification AVA vs Mean gradient across the
valve and to compare it to CT calcium score. And to compare that to the
standards.
What standards will you be auditing against?
BSE echo guidelines and ESC VHD guidelines.
Audit tools to be used
To collect data of 100 consecutive patients from our echo database software
“Echo-Xcelera” by searching for “Severe aortic stenosis” in the study
conclusion, and to review the echo criteria used for AS grading, and to
compare the echo findings to their CT calcium score if present.
Please tick additional reasons (if any) for carrying out this audit:
 Patient centeredness
 Professional development
 High volume activity
 Service improvement
 High risk activity Re-audit
 High cost activity
 Policy/guideline recommendation
Specify if: For proper assessment of AS grading of patients especially before
non-cardiac operations, which might lead to declination of their operations
because of being high risk due to having “severe” aortic stenosis.
Has a literature search been undertaken?  Yes  No
Sample size: 100 patients
Progression of the audit: First cycle done with all it’s phases and will run the
second one to close the loop.
