Objective
The objective of this method statement is to illustrate the Removal Operation of the
Steel Formwork and Suspended Steel Cage.
For SFK Site: Ma On Shan, Hang Kin Street

Scope of work
This removal procedure is applicable for the Steel Formwork and Suspended Steel
Cage.

4.
Responsibility and supervision
a) Site Supervisor
Site Supervisor is the person in charge of the work in works areas.
b) Safety Supervisor
To carry out on-site assessment of the working conditions in the
works area. To make recommendations on measures taken in
relation to safety and health of workers while working in that area.
In addition, to enforce safety works practice and environment in the
workplace and works site.

c) Working Team
The working team is the workers, selected by our Project
Coordinator or his delegate from our employee, who have
completed relevant training.

Suspended Steel Cage
Critical risks
a. Sling and hook are attached to the anchor point on the steel formwork

and suspended steel cage, release the nuts and bolts. Then lift up to its

b. Steel formwork and suspended steel cage arrive at the dismantle area.

Lift down flat on the ground and release the hook from anchor point.

c. Excavator with Hydraulic Scissors will scrape the steel formwork and

suspended steel cage into minor portion.

d. Truck mounted crane pick up the minor portion and discharge to its rear

Safety Aspect
6.1 Specific Safety Induction Training Course
All personnel must attend an induction course conducted by Sun Fook Kong
Safety Supervisor.
6.2 Specified Tool-Box-Talk Training
All personnel will be trained by their supervisors in order to carry out their
works in a safe manner.
6.3 Personnel Protective Equipment
Safety helmet and reflective vest must be worn at all times on site while the
other PPE such as safety boots, gloves, goggles, respirator, hearing protectors,
safety harness, etc. shall be used when necessary.

All PPE must be of approved type and suitable for the user.

6.4
Dismantle Area
Dismantle Area will be fenced off, Site Supervisor/Safety Supervisor must
safeguard no external personal enter the dismantle area. Dismantle area will
be frequently spray with water to avoid dust.
7. Supervision Plan
7.1
Role of site supervisor
The site supervisor carries out an assessment of the work method to be used
and the plant, equipment and materials to be used in work activities. Before
instructing the working team to enter the works space, the site supervisor will
conduct the daily pre-work briefing, arrange necessary safety precautions,
personal protective equipment, and ensure they are in place and adequate. He/
she will check and ensure that the working team members are qualified to
enter the works space and carry out the works. The site supervisor will be
responsible to maintain communication with other working parties in or near
the works space. He/ she will supervise the works. The site supervisor must
ensure the working team has left the works space after completion of the
delegated works.
7.2
Role of safety supervisor
The safety supervisor carries out an assessment of the working conditions for
works and make recommendations on measures to be taken in relation to
safety and health of workers while working in that space. The safety
supervisor reminds the working team to take necessary safety precautions
before commencement of work.
7.3
Role of working team
The working team must follow the instructions of the site supervisor/safety
supervisor at work.
