Overview
Use this form if you are a teacher who has previously resigned or taken leave from a permanent teaching position to care of your own child and wants to
apply for childcare credit.
Note: All mandatory fields are marked with an asterisk (*) and MUST be completed.

Complete fields 1–2. For School to complete.
Complete fields 3–6. This information is used for identification purposes.
Please provide at least one contact detail.
Complete field 13. This information is used for identification purposes.
Complete fields 14-20 for each childcare placement.
School: Enter the school where the applicant resigned or took leave from their permanent position for childcare.
Employment during childcare: Enter the role of the applicant during the period of childcare; starting date of that role,
and date when that role ends. For example, relief teaching or other.
The employee must sign the applicant signature to confirm that all information provided on the NOVO18t form is true and
correct.
The principal must sign the declaration and supply their personal MOE number.
Acting principals can only sign the declaration if they are receiving a higher duties allowance for acting as principal.
A deputizing principal cannot sign the declaration.
* Electronic signatures (typed full legal name, inclusive all middle names) can be accepted
A school Authorised User of Novopay (usually your school administrator or school principal) must email the completed form and supporting documents
(certified true copies scanned in colour) to the Salary Assessment Unit:
