1. INTRODUCTION
OBJECTIVES OF THE DOCUMENT
1.1
The scope of this document is to establish minimum guidelines and responsibilities for issuing/receiving of
work permits and to provide recommendations on how to plan and execute potentially hazardous jobs in a
safe manner. When issued, these permits served as official records of conditions and requirements agreed
upon by the validator and originator. They list minimum safety precautions to be taken and hazards which
must be controlled.
The purpose of this document is to ensure that a safe work environment is achieved by providing
management control over the various work activities carried by Snamprogetti Saudi Arabia Ltd. and
Subcontractors which may be potentially hazardous within the designated fenced areas known as
Construction Areas.
This procedure defines the administration arrangements of the work permit system only for the construction
activities phase and shall not be used for the commissioning activities phase.
However, some construction activities will be carried out in the areas not under the direct responsibility and
control of the Snamprogetti Saudi Arabia Ltd., and then during this phase, it will be managed under the
control procedures of the client.
Where work is to be carried out by Snamprogetti Saudi Arabia Ltd. in areas under the direct control of the
client, this work will be undertaken utilizing the work permit system of the area owner.
1.2
AREA OF APPLICATION
This Work Instruction applies to Snamprogetti Saudi Arabia Co. Ltd.
METHODS OF IMPLEMENTATION
1.3
Upon approval by the CEO, this Work Instruction, supersedes its previous revision (if any), shall take effect
immediately and is applicable to Snamprogetti Engineering & Contracting Co. Ltd. that carry out the activity
described herein.

Work Permit
Is a written form used to authorize jobs that expose workers to serious
hazards. It identifies the work to be done, the hazards involved, and the
necessary preparations and precautions for the job.
Issuing Authority
(Validator)
Person(s) appointed and authorized by the Construction Manager who is(are)
responsible for reviewing the work permit, confirming that the work site has
been examined and all precautions to be taken before work commences.
Receiving Authority
(Receiver)
Person(s) who is(are) appointed and authorized by the Subcontractor
Construction Manager to receive work permits.
Work Permit Coordinator
Person(s) appointed and authorized by the Construction Manager to
coordinate the issuance of all work permits to identify potentially conflicting
work tasks both on geographical and time basis.
Operating Authority
(Operator)
Person(s) who will sign the work permit and verify that all requirements have
been met and that the work area is in safe condition for the work to
commence.

ROLES AND RESPONSIBILITIES
The following entities shall ensure the following responsibilities as herein defined:
3.1
CONSTRUCTION MANAGER
The Construction Manager shall responsible for:
• Ensuring the effective administration of this procedure within construction site activities and shall ensure
that this procedure is properly implemented at site
• Shall ensure that all work is carried out under the controls of Work Permit System
• Ensure that sufficient funds and recourses are made available to ensure the continuity of this procedure
for the duration of the works
• Nominate a Work Permit Coordinator to ensure that the coordination of permits is administered correctly
• Ensure the proper coordination of construction activities between Contractor Construction Manager and
Subcontractor Construction Managers
• Verify that the Subcontractor Construction Manager is providing the correct information and equipment
for the scope of work
• Ensure training is given to all persons required to use the Permit to Work
• Nominate personnel and authorize them to perform the duties of Permit to Work Signatories
• Approve the personnel nominated by the Construction Manager as signatories of the Work Permit
System
3.2
FIELD HSE COORDINATOR
The Field HSE Coordinator will:
• Ensure that all necessary support, advice and guidance are made available to enable this procedure to
be properly implemented
• Ensure adequate training to all personnel involved in this procedure
• Ensure that suitable and sufficient inspections and audits are carried out

Evaluate the effectiveness of this procedure and changes amend as is required or becomes necessary
to achieve its purpose
SUBCONTRACTOR CONSTRUCTION MANAGER
The Subcontractor Construction Manager will:
• Ensure that sufficient and qualified resources are made available to enable the safe implementation of
this procedure
• Ensure that all concerned personnel have read and understood this procedure
• Ensure that adequate facilities and means are in place for the implementation of this procedure
• Identify and communicate in writing the personnel authorized to sign the work permit such as Receiving
Authority and Performing Authority
3.4
SUBCONTRACTOR HSE MANAGER
The Subcontractor HSE Manager shall:
• Ensure that proper instruction, training and supervision are provided at all times
• Ensure that suitable and sufficient inspections are carried out by competent personnel, and that proper
records are maintained
• Ensure that work is carried out in accordance with this procedure
• Ensure that the conditions are satisfactory for the work to proceed
• Ensure that each item on the permit is accurately filled out and covered
• Ensure that personnel understand the requirements on the permit and any unusual conditions
connected with the job
• Ensure the continuous monitoring of the work area conditions and to stop, the environment and/or
equipment to convey pertinent information about the work to the relievers
• Advise the Field HSE Coordinator of any irregularities or areas of concern in order to identify any
necessary corrective action
• Sign the relevant part of the Work Permit where applicable in case of gas test
3.5
SUBCONTRACTOR RECEIVING AUTHORITY (ORIGINATOR)
The Subcontractor receiving authority (Originator) has the following responsibilities:
• To instruct the Subcontractor Performing Authority (Performer) of the permit requirements and satisfy
himself that the Subcontractor Performing Authority has enough information and resource to perform the
work safely
• Ensures that the Subcontractor Performing Authority understands what, where, and how the work is to
be performed
• To ensure compliance with the conditions stipulated on the permit
• To ensure that the Subcontractor Performing Authority supervises the work group and is satisfied that
the persons carrying out the work use the proper tools and equipment for the job
• That the Subcontractor Performing Authority informs the work group of the safe working practices
• To inform the Issuing Authority of any changes in the condition of the job or the surrounding area, or of
any delay in starting the work, so as not to infringe upon any safety factors
• Notify the Issuing Authority upon completion of the work
3.6
ISSUING AUTHORITY (VALIDATOR)
The Validator of the Permit to Work has the following responsibilities:
• To check the completeness and reliability of the submitted forms
• To specify the required worksite precautions and controls
• To detail required safety equipment
• To identify any worksite preparation requirements, if deemed necessary
• To ensure that Certificates (attached to Permits as required) for Confined Space work, excavation and
electrical works are duly signed by authorized engineers

To ensure Gas Test have been completed as required by the Permit
GAS TESTER (SAFETY SUPERVISOR/OFFICER)
Authorized Gas Tester is responsible for:
• Ensuring that the necessary condition monitoring is carried out within the specified time limits and a
record of the results is marked upon the Permit to Work
• Verify the testing has been done
3.8
WORK PERMIT COORDINATOR
The Work Permit Coordinator nominated by the Construction Manager has the responsibility of ensuring that
all permits to work are entered into the system in the correct manner. Additionally the work Permit
Coordinator shall:
• Ensure that all Permits to Work are submitted onetime for entry into the system
• Ensure the Permit to Work is completed correctly and that all relevant controls and signatures are
added
• Permit to Work is properly distributed to the interested parties as indicated; understand the scope and
the details of execution of the work
• Ensure the necessary checks have been satisfactorily executed before signing the permit to work
• Suspend the Permit to Work at any time if the Permit to Work conditions are not complied with
• On satisfactory completion of the works, approve the effective closure of the work and sign for
acceptance
3.9
OPERATING AUTHORITY (OPERATOR)
The Operating Authority is responsible for carrying out all preliminary checks required by the Issuing
Authority and will sign the Permit to Work to verify that all requirements have been met and that the work
area is in a safe condition for the work to commence. The Operating Authority is nominated by the
Construction Manager. Additionally, the Operating Authority shall:
• Ensure that all signatories have signed the Permit to Work
• Ensure that all instructions training has been given to the work party prior to the work commencing
• Inform the Issuing Authority that the work is about to commence in order that this can be notified and
recorded as a live permit
• Inspect the worksite periodically to ensure there is no change in condition
• Suspend the work immediately if there is any breach of this procedure or change in conditions and to
notify Issuing Authority for the reason
• Ensure a good standard of housekeeping is maintained throughout the work and that all equipment is
removed upon completion
• Notify the Issuing Authority upon the completion of work
• Inform the Issuing Authority of any change in the working conditions of the surrounding areas in the
vicinity of the working area
3.10 SUBCONTRACTOR PERFORMING AUTHORITY (PERFORMER)
The Subcontractor Performing Authority is the person responsible for supervising the work group carrying
out the job and shall hold as a minimum the supervisory grade of a Foreman. The subcontractor Performing
Authority is not entitled to start any job without previous approval of the Permit to Work by both Issuing
Authority and Operating Authority.
The subcontractor Performing Authority is responsible to:
• Ensure the work group carrying out the work is fully briefed usually by means of dedicated tool box talk
• Ensure that they have clearly understood instructions given, including the scope of their activity and the
relevant risks
• Remain at the worksite until the completion of the task
• After signing the Permit to Work the Subcontractor Performing Authority shall ensure that all copies of

the Work permits are correctly distributed and that a copy is retained and posted at the work site
Inform the Subcontractor Receiving Authority or Operating Authority of any change in the working
conditions of the job, or an delay in starting the work
Using proper tools and equipment for the safe execution of the job
Ensure the worksite is left in a safe and tidy condition
DESCRIPTION OF ACTIVITIES
4.1
GENERAL PRINCIPLES
The persons involved in the activities regulated by this Work Instruction operate in compliance with the
regulatory and organisational system, and the system of powers and internal proxies. They shall also act in
accordance with the law, applicable regulations and in compliance with the principles established below.
Traceability – the persons involved in the activities regulated by this Work Instruction ensure - each within
their own sphere of responsibility - that the activities and documents pertaining to the process remain
traceable, by seeing to it that the respective sources, information and checks can be identified and
reconstructed. Furthermore, all documents shall be stored, in compliance with current applicable legislation,
using dedicated information systems, whenever available.
Confidentiality – without prejudice to the transparency of the tasks performed and the obligations of
disclosure imposed on applicable provisions, including those relating to the keeping and updating of lists of
persons with access to privileged information, all persons involved in the activities regulated by this Work
Instruction are under obligation to ensure the appropriate degree of confidentiality for all information that
may come to their attention by virtue of their position.
Segregation of Duties – in the activities regulated in this Work Instruction, a segregation of duties and
responsibilities must be provided, in order to prevent situations in which activities are concentrated on
specific parties, which may contribute to creating conditions of risk with regard to the reliability of the
information and the correct performance of duties. In dividing/assigning the activities, incompatible duties
between and within the functions shall be segregated in accordance with the segregation principles
prescribed by the internal control system applicable to the different entities. In practice, the principle is
applied in relation to the nature of the activity concerned and the type and degree of risk associated with it,
with a view to preventing the formation of unduly cumbersome organisational structures, especially in
entities of moderate size.
Conflict of Interest – The relationship of personnel involved in the activities regulated in this Work
Instruction with their counterparts shall aspire to the highest standards of ethical behaviour in compliance
with the Saipem Code of Ethics. Every effort must therefore be made to avoid all situations and activities
which could give rise to a conflict with the company’s interests or which could interfere with a person’s ability
to take decisions impartially in the best interests of the company and in full observance of the principles and
contents of the Code of Ethics and of the company’s Model, the Anticorruption MSG and, in general, with
the correct performance of their duties and responsibilities. Every situation that may constitute or determine
a conflict of interest shall be handled in accordance with the Code of Ethics and with the Anticorruption
MSG.
Anti-corruption Policy – Saipem prohibits all forms of corruption, without exception. In particular, it is
forbidden for Saipem personnel to (a) offer, promise, give, pay or authorise anyone to give or pay, directly or
indirectly, a financial or other benefit to a Public Official or private citizen; (b) accept a request or demand, or
authorise someone to accept the request or demand, directly or indirectly, for anything of value or other
utility from a Public Official or private citizen, when in both cases the intention is (i) to induce a Public Official
or private citizen to perform improperly any function of a public nature or any activity connected with a
business or to reward them for the improper performance of such a function or activity; (ii) to influence an
action or omission by a Public Official or any of the Public Official’s decisions in violation of official duty; (iii)
to obtain, secure or retain business or an improper advantage in the performance of business activities; or
(iv) to violate current laws in any way.

Value creation/maximization – all of Saipem’s activities shall be aimed at increasing, in the long term, the
value of Saipem’s assets and of its managerial and technological skills and knowledge, as well as at
creating value and welfare for all stakeholders
Transparency – the persons engaged in the activities regulated by this Work Instruction shall operate in
such a way as to guarantee the utmost transparency in the performance of their duties and to provide full
and truthful disclosure
Continuous Improvement – Saipem assures the continuous improvement of its Processes through a
suitable monitoring system. The functions shall promote the monitoring, development and dissemination of
the know-how relating to the activities regulated by this Work Instruction. In order to guarantee continuous
improvement, performance must be monitored through suitable monitoring and reporting tools
Satisfaction of Customer Departments/Functions – The functions involved in the in the task governed by
this Work Instruction must enable the achievement of the required performance in terms of reliability,
availability and fulfilment of the expectations of customer functions through cooperation and continuous
alignment with them, in compliance with the expenditure limits defined by Saipem. Understanding the
requirements and needs expressed, the constant monitoring of work performed, and feedback of the
effectiveness of the services supplied is the main tools to allow maximization of the satisfaction of the
customer departments
5
OPERATING PROCEDURE
5.1
FUNCTION OF THE WORK PERMIT SYSTEM
Work permits must be issued, as appropriate for any of the work or combination of the work on specific work
on a specific site identified below:
5.1.1
COLD WORK (GENERAL WORK PERMIT)
Any work within the construction area of work that will not produce sufficient energy to ignite flammable
atmospheres/materials. The Work Permit shall be utilized for all activities other than those capable of
generating or using sources of ignition. Use form no. FORM-SPSA-HSE-030-E for cold work permits (PALE
BLUE BORDER).
5.1.2
HOT WORK
Any work within the construction area of work, wherein the heat generated is of sufficient magnitude and
intensity to cause ignition of any flammable liquids, gases or any other material. Common sources of ignition
are internal combustion engines, gas cutting torches, welding machines, non-flame proof electrical
equipment, grinders, annealing blankets, jack hammers, etc. Form no. FORM-SPSA-HSE-031-E shall be
used for hot work permit (RED BORDER).
5.1.3
CONFINED SPACE ENTRY
Entry of personnel into any space or structure (i.e., tank, vessel, vault, excavation when 1.2 meter or deeper
not normally intended for human occupancy) where entry, movement within, or exit is restricted. Use form
no. FORM-SPSA-HSE-032-E (LIGHT GREEN BORDER).
5.1.4
RADIOGRAPHY WORK
To control all site radiography and the use of ionizing radiation, form no. FORM-SPSA-HSE-033-E (LIGHT
ORANGE BORDER) shall be used.

5.1.5
EXCAVATION WORK
Form no. FORM-SPSA-HSE-034-E (BROWN BORDER) shall be used for any excavation jobs.
5.1.6
ELECTRICAL WORK
An electrical permit (Form no. FORM-SPSA-HSE-035-E) shall be used for any activity to be carried out on
live electrical equipment or system.
5.1.7
GRATING HANDRAIL REMOVAL AND OPEN HOLE CERTIFICATE
A grating, handrail and open hole certificate (Form no. FORM-SPSA-HSE-039-E) shall be used for any
activity that requires the opening of gratings and/or removal of handrails on existing final structures;
5.1.8
LIFTING CERTIFICATE
A lifting certificate (Form no. FORM-SPSA-HSE-040-E) shall be used for any lifting activities that are to be
carried out.
5.2
5.2.1
ISSUANCE AND APPROVAL OF WORK PERMIT FORMS
BEFORE ISSUING THE WORK PERMIT
All necessary actions to improve the proposed work and/or possible configuration changes must be
completed. The issuer must verify that the receiver is experienced, competent and familiar with the
requirements of the Work Permit system and be able to perform the duties associated with receiving work
permits.
5.2.2
VALIDATOR AND ORIGINATOR
The validator and originator are jointly responsible for the safety of personnel and equipment in the area.
The validator is responsible for specifying precautions so that the work can be done without injury or loss.
The originator is responsible for performing the work in accordance with the conditions and requirements
agreed upon with the issuer and specified in the work permit.
5.2.3
WORK PERMIT RECEIVER AUTHORITY (ORIGINATOR)
The originator requests the work permits from the work permit validator and complete the initiation part of
the work permit form. The originator must participate in any joint site inspection that is conducted.
5.2.4
WORK PERMIT ISSUING AUTHORITY (VALIDATOR)
The validator will issue the work permit after he has conducted a joint site inspection with originator as
appropriate, performed the necessary gas tests, listed applicable precautions and restrictions, and reviewed
all aspects of the job with the receiver. The validator and originator signatures plus any other approvals and
signatures must be on the completed form.
5.2.5
WORK PERMIT PERFORMER
The Subcontractor performing authority must keep a copy in view at the job site for the duration of the job so
that it is available for inspection and cancellation if necessary. The receiver is expected at the job site to
adequately supervise the work. If he must leave the jobsite for any reason, the issuer concurs. The alternate
receiver must be written on the permit by the issuer.

5.4
RIGHT TO STOP WORK AND CANCEL PERMITS
The issuer may stop and withdraw a work permit if he determines that safety of the job does not meet
the conditions specified on the work permit
When work is stopped the issuer must pick up the work permit, write on the permit the reasons for the
work cancellation, the time, and other relevant information
When the conditions are safe again the issuer may issue a new work permit
The receiver has the responsibility to stop work and advise the issuer of his action any time he
determines that the conditions of the job do not meet the conditions on the work permit. He and the
issuer shall take action to correct deficiencies and resume the job
All necessary personnel whose presence adversely affects the safety of the job must be kept away from
the job site or work shall be stopped
All work permits are cancelled in the event of an emergency, such as a fire or toxic release. Permits
may be re-issued once the “all clear” has been declared and the work sites have been returned to their
pre-emergency conditions
If an unsafe condition develops when neither the validator nor the performer is available at the work site,
a performer’s delegate has the authority to stop the job
CLOSING WORK PERMITS
Work permits shall be closed by both the operator and the performer signing the permit. Both the operator
and performer shall conduct a site inspection to ensure that the work area has been returned to its pre-work
condition before the work permit can be closed. If the work has not been completed, the work site shall be
left in safe and secure condition.
5.5
FILING WORK PERMITS
The issuer shall keep the original copy of the work permits for a minimum of three (3) months.

UPDATING
The functions/departments and the positions involved in the activities regulated by this Work Instruction are
responsible for monitoring Company operations that entail the need to update it. These findings are reported
to HSE Department that ensures coordination of the activities for updating this document.

ARCHIVING, DOCUMENT STORAGE AND TRACEABILITY
The functions / departments and the positions involved in the activities regulated by this Work Instruction
shall ensure, each for matters under its responsibility and also by means of the information systems used,
the traceability of data and information and shall provide for storing and archiving the produced hardcopy
and / or electronic documentation, in order to enable the reconstruction of the different steps of the process.
