OCCUPATIONAL SAFETY AND HEALTH PROCEDURE ACCIDENT AND NEAR MISS REPORTING AND INVESTIGATIONS JKKP-ILJTM REVISION NO. : 0 RPD NO. :0 EFFECTIVE DATE : PAGE : 3 from 6 3.0 RESPONSIBILITIES 3.1 All staff and customers must be responsible for reporting accidents, dangerous incidents, occupational poisoning or occupational diseases that occur within ILJTM. 3.2 The Safety and Health Committee shall be responsible for implementing accident report and investigation procedures. 4.0 5.0 DEFINITION AND TERMINOLOGY 4.1 SHO - Safety and Health Officer 4.2 OSH-COMITTEE - Occupational Safety and Health Committee 4.3 DOSH - Occupational Safety and Health Department 4.4 ILJTM - Human Resources Department Training Institution 4.5 OSH ACT - Occupational Safety and Health Act and Regulations PROCEDURE 5.1 ACCIDENT REPORTING ACTION 5.1.1 Every accident, dangerous incident of occupational poisoning or occupational illness must be reported to the Director of the Manpower Department throughthe Manpower Department / Manpower Department Industrial Training Institute Safety and Health Committee. The report should be made as soon as the incident is identified to the respective Manpower Department / Manpower Department Industrial Training Institute. 5.1.2 In the event of a hazardous event, as set out in the Second Schedule, at any place of employment, the Manpower Department / Manpower Department Industrial Training Institute shall notify the JKKP as soon as possible within 7 days to submit a report on the accident using the Accident /Incident Report Form (JKKP 6). OCCUPATIONAL SAFETY AND HEALTH PROCEDURE ACCIDENT AND NEAR MISS REPORTING AND INVESTIGATIONS JKKP-ILJTM REVISION NO. : 0 RPD NO. :0 EFFECTIVE DATE : PAGE : 4 from 6 5.1.3 Accidents that cause death and serious bodily injury and are unable to attend work for more than four (4) calendar days, Manpower Department / Manpower Department Industrial Training Institute must report to the Occupational Safety and Health Department within seven (7) days using the Occupational Poison Report / Occupational Illness (Occupational Safety and Health Committee 7) through the Manpower Department / Manpower Department Industrial Training Institute Occupational Safety and Health Committee. 5.1.3.1 All complaints and minor accidents occurring in Manpower Department Manpower Department Industrial Training Institute should also be recorded in the Accident, Dangerous Incidents, Occupational Poisoning / Occupational Disease (JKKP 8) form and forwarded to the Department of Occupational Safety and Health for further action. 5.1.3.2 All the parts that need to be filled out in the form need to be completed in full including a description of the accident and complaint for prevention, so that the same incident will not be repeated. 5.1.4 In the event of an accident that causes serious injury, the location of the accident shall not be tampered with until investigation by all parties including officials from the Department of Occupational Safety and Health is completed. The advice and views of the PKK or the Deputy Director General of Manpower Department / Director of Manpower Department Industrial Training Institute and related parties should be obtained immediately.
