You are completing your post-op assessment of Mrs. Doyle and she is complaining of 5/10 pain
to her left hip incision area. She states the Morphine made her very nauseated.
You Complete your assessment and then prepare to administer her some IV Gravol for nausea
and possibly more Morphine for her nausea and pain.
While preparing the IV Gravol and Morphine you see that the order is incomplete. You need to
clarify the Morphine order with the physician.
Using SBAR what are 10 pieces of information that you would present when asking for the clarification
order. You may list them in any order, but in the proper category, using the chart below and submit into

I am Sinthia, SN on 4 West unit.
I am calling about Patient Mrs, Doyle. I am calling because the IV
Gravol and Morphine order is incomplete. Patient states she is
nauseated by last Morphine administration. I am concerned that she
was given a wrong dose.
Mrs. Doyle is a Post – Op patient
She states 5/10 pain to left hip incision area,
She stated she felt nauseated by Morphine administration.
I think the problem is the drug type that was administrated or the
dosage as the order is incomplete.
I need you to come see the patient and review the order so that the
proper pain medication be prescribed to Mrs. Doyle to avoid further
complications.
Is there anything I need to do in the meantime?
