- and The WEST VISAYAS STATE UNIVERSITY through its College of
Medicine, Extension Unit with principal address at Luna Street, La Paz, Iloilo City,
herein represented by DR. JOSELITO F. VILLARUZ, University President, herein
referred to as the Second Party.
- WITNESSETH WHEREAS, the FIRST PARTY, as a religious institution, recognizes its vital
role in the encouragement to uplift the level of well-being of the Filipinos;
WHEREAS, the FIRST PARTY is missioned for SOCIAL SERVICES
DELIVERY to help the poor, disadvantaged, vulnerable, and marginalized
individuals, families, and communities;
WHEREAS, the FIRST PARTY acknowledges the significance of interagency collaboration in the delivery of health and related services to address the
diverse needs of the growing population of the community;
WHEREAS, the SECOND PARTY, as an agency promoting health and
wellness;
WHEREAS, the SECOND PARTY is mandated to perform EXTENSION as
one of the trilogy of functions of the highest institution of learning;
WHEREAS, the SECOND PARTY can provide the expertise needed in the
effective implementation of Health Services anywhere in the Philippines;
WHEREAS, the SECOND PARTY commits the necessary qualified and
competent personnel for the provision of health and related services in order that the
desired outcomes of the activity are achieved;
WHEREAS, for and in consideration of the foregoing premises and mutual
covenants and agreements herein after set forth. the parties hereto have agreed as
follows:

Provide a venue for the provision of health and other services.
Facilitate the delivery of needed assistance through collaborative efforts with
partner institutions.
Present objective evaluation of the initiative.

Offer health and other services to uplift the status of the community.
Establish linkages with other health providers to ensure better delivery of aid
needed in particular circumstances
Identify communities needing assistance and facilitate provision of needed
services.
Conduct monitoring and evaluation of initiatives for future refinements.
EFFECTIVITY, RENEWAL, AMENDMENT AND TERMINATION
The Parties agree that this Memorandum of Understanding is not a formal
legal agreement giving rise to any legal relationship, rights, duties or consequences,
but is only a definite expression and record of the parties to which they are bound in
honor only.
This Memorandum of Understanding is good for one year and its effectivity
starts on the date when both parties sign if. Any amendment which shall be embodied
in an appropriate written instrument shall be subject to mutual consent and approval
of both parties
