Case Presentation
Instructions: You will use the following outline to guide your case presentation. You may
complete the form and share it with your classmates or you may present it in PowerPoint form.
At the end of the case presentation, you will play your recording according to question 9.
1. Demographics
 What are the relevant demographic variables for the student (e.g. age, grade level,
SES, race, sexual orientation, gender identity, disability status, immigration status,
language, size, religion, living situation, etc.)?
2. Presenting Problem
 What is the student’s main issue?
3. Secondary Problems
 What other problems is the main issue causing and in what other areas of the
student’s life? Consider the ABCs (i.e. attendance, behavior, and course
performance); family; social life; emotional health, etc.
4. Contributing Factors and Other Relevant Information
 Are there any other factors contributing to this student’s problem, such as family
dynamics, teacher-student dynamics, cultural background, values, history,
medication, etc.?
 Is there anything else we should know about this student, e.g. test or assessment
results, past history of substance use, suicidal thinking, risky behaviors, etc.?
5. Resources
 What are the student’s strengths, resources, and coping skills?
6. Conceptualization
 What do you think is happening with this student? Discuss this from a theoretical
perspective.
 What stage of change is this student in and why (e.g. Precontemplation,
Contemplation, Preparation, Action, or Maintenance)?
7. Goals and Plan
 What are the goals for this student?
 How will you and the student collaborate to solve those goals?
 Discuss the interventions you will use and keep in mind your theoretical orientation
from the conceptualization.
 How will you assess whether or not the student has met those goals?
 Are there any other stakeholders that need to be involved (e.g. teachers, principal,
community partners, family, etc.)?
8. Ethical and Legal Considerations
 Are there any ethical or legal issues with this case?
9. Assessment of Counseling Session & Tape Review
 Identify a skill you used or something you did well with your student.
 Identify an area of difficulty, confusion, or something you have a question about.
 Be sure to present a part of your tape that covers something you want to discuss with
your supervision group and something you discussed in this section of your
presentation.
 Your session should be that of a typical session (e.g. 20 minutes) and the part you
share in your presentation should be at least 5 minutes.
