AFFIDAVIT OF LOSS
I, CORAZON ORPILLA GAZCON, of legal age, married, Filipino
citizen, and a resident of Centro, Baggao, Cagayan, after having been duly
sworn to in accordance with law, depose and state:
1.
That I am the holder and in possession of an Original Certificate
of Title No. P-10950, which cover a parcel of land consisting of an
area of 81,054 square meters, located in Taguing, Baggao, Cagayan
(Lot 1158, PLS 492-D), which was acquired by our late father,
FRANCISCO C. ORPILLA SR., by virtue of a Deed of Sale dated
May 1, 1964, duly recorded as Doc. No. 182, Page No. 40, Book No.
IX, Series of 1964 in the Notarial Registry of Cipriano S. Verbo,
Notary Public of Tuguegarao, Cagayan;
2.
That however, the said title was destroyed when it was
unfortunately flooded in our ancestral house at Centro, Baggao,
Cagayan, during the onslaught of the super typhoon “Ompong,”
on the evening of September 15, 2018;
3.
That by reason of the above fact and circumstance, and
considering that the said title is already beyond recovery, it is
considered as lost for all intent and purposes;
4.
That as a surviving heir of the late FRANCISCO M. ORPILLA, SR.,
I am executing this Affidavit of Loss for the purpose of Reissuance
of Title by the Register of Deeds in accordance with Section 109 of
P.D. 1529 or the Property Registration Decree;
5.
That I am executing this Affidavit of Loss to attest to the
truthfulness of the foregoing and for whatever legal intent and
purposes this may serve.
