INSTRUCTIONS FOR USE: Please use this as a template deleting any instructions
and items that do not apply to you or your child and remove the highlights of these fields.
Please be certain to read through all instructions for the form and download the G-1145
form (for notice of processing). https://www.uscis.gov/n-600. This template assumes all
adopting parents were US citizens at time of adoption who were born in the US. N600
instructions will provide details if this is not the case about varying supporting
documentation that is needed.

Dear Madam or Sir:
Enclosed is a completed form N600 in support of my Application for Certificate of
Citizenship on Behalf of Adopted Child.
Parent Name: (only use the name of whomever filled out the form)

Also enclosed is the following supporting evidence:
1. Form G-1145 requesting notification of processing
2. Two identical passport style 2x2 photographs of CHILD taken on DATE (within
30 days of filing, add name and alien registration number in pencil on back). See
N600 instructions page 2) for more details about specifications of photos
3. Photocopy of CHILD’s foreign adoption decree (delete if child is from the
Philippines or South Korea prior to Korea issuing decrees)
4. Photocopy of Readoption Certificate or proof of legal name change
5. Photocopy of CHILD’s Permanent Resident Card
6. Photocopy of birth certificate or identifying page of US Passport of Child’s Mother,
a U.S. citizen
7. Photocopy of birth certificate or identifying page of US Passport of Child’s Father,
a U.S. citizen
8. Photocopy of front page of CHILD’s foreign passport issued by COUNTRY in the
name of CHILD’s NAME ON PASSPORT (Child’s former name)
9. Photocopy of page stamped by US Immigration with visa designation (IR3, IR4,
IH3, IH4)
10. Photocopy of certified copy of Marriage Certificate of Child’s Parents (delete if
not applicable)
11. Photocopy of certified copy of Divorce (or death) Decree of Child’s Parent (delete
if not applicable)
12. Photocopy of driver’s license of children’s parent(s)
13. Proof of required residence or physical presence in the US (see page 3 of N600
instructions)
14. A money order/check in the amount of $550 (if filed before 12/23/16) payable to
the Department of Homeland Security

The following documents are contained in the original immigration file for my child and
should be used in support of this application:


Child’s Foreign Birth Certificate with certified translation
Child’s Foreign Final Adoption Decree with certified translation
Copies of documents submitted are exact photocopies of unaltered original documents,
and I understand I may be required to submit original documents to an Immigration or
Consular official at a later date.
