Once the locks and tags are
applied the PI will ask the
isolation & the operation
personnel to bring equipment
/ machine / process / system
to zero energy state as per
the Step-6 of LOTO process.
Upon confirmation of zero
energy state the PI will or may
ask the operation personnel
to try out. Once satisfied the
PI will issue the PTW as per
the Step-7 of LOTO process.
The ME will assess the needs
for the job and isolation. Then
they will fill out PTW and the
LOTO
Implementation
Checklist as per the Step - 1 of
the LOTO process.
Once the isolation is done the
PI will ask the isolation
personnel and others who will
be on the job to apply there
individual locks & tags as per
the Step - 5 of the LOTO
process. (Also Alternate LOTO if
required)
Upon receiving the PTW the
ME will supervise the
execution person (s) and
perform the task as per the
Step - 8 of the LOTO process.
Once need for isolation is
identified, the 1st section of
LOTO
implementation
checklist i.e. "Energy details"
will be filled in by the
executive themselves.
The Isolation personnel will
turn off / shut down the
energy sources and isolate the
energy
sources
to
the
equipment / machine / process
/ system as per the Step - 3&4
of the LOTO process.
Upon completion of the job the
ME will approach the PI to close
the permit. The PI will inspect
the job site to ensure all
personnel & tools are away
from the job site as per the Step
- 9 of the LOTO process.
After filling the PTW and the
LOTO
Implementation
checklist, the ME will request
the PI to issue the Permit To
Work (PTW).
The PI will verify the details
on the PTW and the LOTO
Implementation Checklist and
will inform the affected
people & will call for suitable
isolation personnel as per the
Step - 2 of the LOTO process.
Once the inspection is done
the PI will ask the isolation
personnel to restore the
energy and the PTW will be
closed thereafter as per the
Step-10 of the LOTO process.

If a scheduled preventive
maintenance is due.
They can do audit when any
job where LOTO is applied is
being carried out using the
LOTO audit checklist by
visting the site, checking the
documents & by interviewing
people.
Authorized LOTO Auditors will
conduct audit to check
effectiveness of the LOTO
process.
They can also do audit when
no job applying LOTO is
happening by using the LOTO
audit checklist, by checking
the old documents and
records and by interviewing
persons who were involved in
the audited instance.

If the auditor (s) find
everything in place and
effective they will mention
the same in the audit checklist
and close the audit process.
If the auditor (s) find gaps in
the process they may ask the
ME or the PI to act on it and
continue doing the job. If any
major gaps observed, then
the PTW will be cancelled.
If the auditor (s) find
everything in place and
effective they will mention
the same in the audit checklist
and close the audit process.
If the auditor (s) find any gaps in
the process, they will raise it to site
management to act on, in case any
major gaps observed no activity
requiring LOTO should be allowed
to happen till all such gaps are
addressed.
