I, MOHD WAZIR ALAM S/O MOHD SADIQ ALAM R/O WZ-H-62 SANT NAGAR EXTN, TILAK
NAGAR, WEST DELHI, DELHI-110018, do hereby solemnly affirm and declare as under:-
1. That the exact and correct date of birth of my child namely (MOHD MEHRAJ) is
16/03/2015 at WZ-H-62 SANT NAGAR EXTN, TILAK NAGAR, WEST DELHI, DELHI110018. The mother name of above said child is Smt. SAKILA KHATOON.
2. That I have not obtained any birth certificate of my child form any Government
agency department in India.
3. That the all supporting documents are genuine and nothing has been concealed.
