(Checklist : Border enforcement measures)
All passengers who travel to Japan are required to complete the following travel
requirements.
1
Complete the required details on the “Written Pledge”
□ I will complete the “Written Pledge” and submit to the airport quarantine station
once I entered to Japan.
2 Confirm your email address and your mobile phone.
Please download MySOS and register your account , set your smartphone to save
location information and install COCOA prior to entering Japan. Required operating
system (OS) versions:iPhone: iOS 13.5 or later. Android: Version 6.0 or later.
□ I have an email address that send and receive are enabled on my mobile phone.
(Confirmed by quarantine office staff at Japan airport.)
□ I already Installed COVID-19 Contact-Confirming Application (COCOA).
□ I already Installed and signed up for MySoS (Video call app).
□ I already completed the account registration of MySos.
□ I already set up my smartphone to save location information. (Settings on
Google Maps, etc)
□ I will complete the medical questionnaire on my mobile phone, Once I receive
the seat number of my boarding flight. (Should be done before entering to
Japan Airport)
3 Other requirements to be practiced to enter Japan
□ I will obtain PCR test negative result that is provided in a specified format for
Japan within 72 hours prior to departure.
□ When I arrive at Japan Airport, I will not use Public transportation including
domestic flights except the case using public transportation to get to the
location of self-quarantine within 24 hours from the PCR test done at airport.
□ I will be quarantined at the accommodation provided by the chief of the
quarantine center or be quarantined at home if received the booster vaccine.
□ If I don’t have any smartphone which cannot install above APPs, I have an
international credit card to borrow a smart phone in Japan.
