Transfer is effective as of the date set forth above. 4 Paws 4 Life Rescue C/O Toni Barge, Tina
Miller 3648 N Perry Park Rd, Sedalia, CO The receiving organization (“Transferee”)
acknowledges receipt of disclosure of heartworm status of dog based upon the heartworm
completed at other vet or shelter. Additionally, Transferees acknowledges receipt of other health
and medical information listed in the kennel cards for such dog or cat, emails and other
correspondence from Transfer Organization listed above (“Transfer”). Transferee acknowledges
that Transfer DOES NOT MAKE ANY GUARANTEES REGARDING THE AGE, NATURE,
TEMPERAMENT, OR HEALTH OF THIS PET, INCLUDING ANY UNDISCLOSED
MEDICAL CONDITIONS. Transferee assumes all risks and responsibility including the
financial responsibility of ownership of the Pet(s) once they are in Transferee’s possession and
that Transfer is not in any way responsible for such costs incurred. Additionally, transferee
acknowledges that all animals transferred through Transfer are from a heartworm endemic area
and that even dogs that test negative for heartworm disease may be infected. All dogs adopted
from this region should be immediately placed on a heartworm prevention program and should
be tested for heartworm disease six months after transfer.
