Is combustible scrap, debris and waste stored safely and removed from the worksite promptly? Is combustible dust cleaned up with a vacuum system to prevent the dust going into suspension? Are all oil and gas fired devices equipped with flame failure controls that will prevent flow of fuel if pilots or main burners are not working? Are all spilled materials or liquids cleaned up immediately? Is metallic or conductive dust prevented from entering or accumulation on or around electrical enclosures or equipment? Clearance of permit The work for which this permit to work was issued is now completed/suspended and all people under my charge have been withdrawn and warned that it is no longer safe to work on the apparatus detailed in this permit to work. All work equipments, tools, test instruments etc have been removed.
