Course Objectives:
1. Attendees will understand the definitions of Change of Condition per OAR 411-054-0040
and the OSBN definition of Assessment per Division 45.
2. Attendees will be more informed on the most common changes of condition that are
impacting our seniors residing in ALF/RCF facilities.
3. Attendees will become more familiar with resources and references available to help
navigate changes of condition in their community.
4. Attendees will follow DHS Framework for Change of Condition.
5. Attendees will use critical thinking to evaluate their current COC processes and systems
internally to ensure all regulations, scope of practice, and quality initiates related to
oversight, monitoring, and evaluation criteria are met.
