Senior Portfolio Name_________________________ This is to be completed as a Slides/PowerPoint presentation o Title page: title, name, and motto. o Write a personal mission statement. A personal mission statement is a belief declaration, comment, or affirmation of your plans. It answers questions such as; what is my purpose? What is my main goal? How will I accomplish that goal? When do I plan to accomplish that goal? What is my current focus towards that goal? What am I working on at this time? Who are you helping in life? o Birth and appearance: details surrounding your birth. o Family: who do you consider as your family (siblings, relatives). Describe each person and their significance. o Friends: how and when did you meet, what do you do together, and what things do you have in common? o Describe your values. Values are those things that really matter to each of us… The ideas and beliefs we hold as special. Caring for others, for example, is a value; so is the freedom to express your opinions. Where did your values come from? For example, did you get your values from your parents, political figures, celebrities, or friends? o What makes you who you are? This can be a multi-page essay that is part of your presentation if you are a “writer”. This must include but is not limited to: Events and people/role models who affected your life. Your favorite memories of high school. Favorite books, movies, TV shows, food, and pictures. Creativity or talents- what are you good at? Pet peeves- things that annoy you and why? Passions- what are you willing to fight for and why? Describe your thought process? How do you make decisions? How do you think through important matters? What are your fears for the future and how can you overcome them? Is there anyone who could help you? o See autobiography questions for more ideas. o Personal shield- what defines you as a person? Must include a paragraph describing each picture and what it means to you. See your packet for examples. o Who am I poem? Use the template included in this packet. o Write a Thank you letter to someone who has helped you along your journey to get to this point. o Create a Resume, Examples and templates attached o Fill out a job application. Can be done online o “A Look into My Future”- See directions attached o Graduation speech, 1-3 minutes in length.
