1. Line Break Permit Process Only trained Maintenance employees and contractors are permitted to perform a Line Break The exact location of the break in must be identified and marked with Yellow Caution tape Prior to a Line Break, a permit must be completed which o Identifies hazards associated with the Line Break which exist or potentially exist o Identifies control methods for hazard elimination or reduction o Identifies required PPE o Confirms communication with affected employees or contractors o Confirms that applicable control methods have been implemented The permit must be approved by o Maintenance Manager or Process Engineer, and o EHS Engineer or Director of Operations The permit approver must go to the point of the break to confirm all permit conditions have been met The completed permit must be posted near the Break Upon completion of the break and associated job tasks, o use the proper procedure for joining the lines, o remove barricades, supports and grounds, o notify affected persons of work completion, o receive authorization to remove the lockout/tagout, and o restart the equipment. the completed permit shall be placed in the drop box in the Foreman’s office
