Five workers were murdered when the driller's cabin on the rig floor caught fire. They died as a result of thermal burns as well as smoke and soot inhalation. About three and a half hours after withdrawing drill pipe ("tripping") from the hole, the blowout occurred. The failure of both the main barrier hydrostatic pressure produced by drilling mud and the secondary barrier human detection of influx and activation of the blowout preventer, both of which were meant to prevent a blowout, caused the explosion and rig fire. Poor Barrier Management - During the tripping procedure, the equipment was oriented differently than usual, causing uncertainty. Underbalanced Operations Performed Without Proper Planning, Procedures, or Needed Equipment Underbalanced drilling was performed without needed planning, equipment, skills, or procedures, thus nullifying the planned primary barrier to prevent gas influx. 3 . Alarm System Off - The day and night drillers both turned off the complete alarm system, resulting in both drillers going missing. Crucial signs of a gas invasion and a potential blowout In addition, the alarm system was ineffective.
