INFORMATION ABOUT THE ACCIDENT
Date of Accident: January 28, 2018
Location of Accident: Pittsburg country , Oklahoma
Description of the Accident:
A blowout and rig fire happened on January 22, 2018 at Pryor Trust 0718 gas well
number 1H-9 in Pittsburg County, Oklahoma. Five workers were murdered when the
driller's cabin on the rig floor caught fire. They died as a result of thermal burns as well as
smoke and soot inhalation. About three and a half hours after withdrawing drill pipe
("tripping") from the hole, the blowout occurred. The failure of both the main barrier
hydrostatic pressure produced by drilling mud and the secondary barrier human detection
of influx and activation of the blowout preventer, both of which were meant to prevent a
blowout, caused the explosion and rig fire.
HAZARD ANALYSIS
What were the potential hazards that caused the accident? Name at least 3 and write a brief

how it could’ve contributed to the accident.
1. Poor Barrier Management - During the tripping procedure, the equipment was oriented
differently than usual, causing uncertainty. Reading well data, causing rig workers to miss
signs of gas influx.

2. Underbalanced Operations Performed Without Proper Planning, Procedures, or Needed
Equipment Underbalanced drilling was performed without needed planning, equipment,
skills,
or procedures, thus nullifying the planned primary barrier to prevent gas influx.
3 . Alarm System Off - The day and night drillers both turned off the complete alarm system,
resulting in both drillers going missing. Crucial signs of a gas invasion and a potential blowout In
addition, the alarm system was ineffective. Meant to warn people of potentially dangerous
situations during various operating states (e.g., drilling, tripping, etc.) circulation, and surface
activities) during the 14-hour period, and would have sounded an excessive number of non-critical
alarms. Resulting in the blowout, and the drillers' decision to switch off the alert system;
