Results from this evaluation indicate significant concerns with _____’s cognitive abilities. Results from cognitive testing indicate that _____’s IQ falls within the Extremely Low range, with a FSIQ score of 64. He displayed varying abilities, with his highest ability being Visual Spatial and his lowest being Processing Speed. These abilities are crucial for day to day and academic functioning. Individuals within the extremely low range often have significant difficulty in academics, as they struggle to engage in grade-level work. This is the case for _____, who appears to be functioning in the 2nd to 4th grade level with his reading, math, and spelling. This is a significant difference compared to his current placement of 9th grade. To further assess this impairment, adaptive skills were also assessed. Testing indicates that _____’s adaptive skills fall within the extremely low range with a General Adaptive Composite Score in the extremely low range. He demonstrated deficits in nearly every domain. This indicates that _____ is not functioning within the 16-year-old range. An additional neurological measure provided evidence of low cognitive abilities, and further revealed issues with graphomotor skills, visual motor functioning, and visual integration. These results may be indicative of developmental or neurological concerns and suggest concerns with _____’s frontal lobe. The frontal lobe is also responsible for skills related to executive functioning. Executive functioning skills play a role in memory, managing emotions, decision making, and self-control. Based off of these results, _____ meets the criteria for Intellectual Disability, moderate. _____’s low cognitive and executive functioning skills play a large role in his mental health and behavioral outbursts.
