Essential Documentation
The documents enclosed are as follows:
1. DBS List of Documentation – This is a list of all the acceptable documentation that you can
provide to complete your DBS check. As a Trust we aim to follow the DBS Route one, so please
can you try and provide documents from this route. If for any reason you are unable to do
this then please contact your named HR Officer for further advice.
2. Right to Work List of acceptable documents – This is a list of all the acceptable documentation
that you can provide to complete this check. If for any reason you are unable to do this then
please contact your named HR Officer for further advice.
3. Employee Personal Information Questionnaire – Please complete all this information.
4. Health Declaration Form – Please complete this form and provide as much information as you
can if you have a medical condition you need to inform us about.
5. Bank, Tax and Essential Information Form – Please complete this form. There must be an
original signature on this document, electronic signatures cannot be accepted.
1. DBS List of Documentation
In order to undertake this essential pre-employment check, it is necessary to see 3 original documents which prove your
identity and current home address.
The documents required depend on the route the application takes. You must always try to provide documents from
Route 1 first.
Route 1
You MUST be able to show:
1 document from Group 1, below
2 further documents from either Group 1, or Group2a, or 2b, below
At least one of the documents must show your FULL name and current address.
Where any documents are in a different name to your current name i.e. due to marriage, then a marriage certificate or
proof of name change deed MUST also be provided.
Route 2
If you don’t have any of the documents in Group 1, then you must be able to show:
1 document from Group 2a PLUS
2 further documents from either Group 2a or Group 2b
At least one of the documents must show the applicant's current address
You MUST inform the HR Officer ASAP if you are unable to provide the necessary documentation to fulfil Route 1 or
Route 2, prior to moving to Route 3.
Route 3
This Route will only be used in circumstances once it has been fully explored with you why your identity has not been
validated following either Route 1 or Route 2. This will include holding a probing discussion with you about the likely
reasons why your identity has not been validated.
For Route 3 you must be able to show:



A certified copy of a UK birth certificate; PLUS
4 further documents from Group 2 comprising of:
1 document from Group 2a; and
3 further documents from Group 2a or 2b; one of which must verify your current address

(Following documentation should be issued within the past 12 months)

Financial Statement e.g. pension, endowment
P45/P60 (UK & Channel Islands)

Following documentation can be more than 12 months old bust MUST still be valid

Cards carrying the PASS accreditation logo (UK & Channel Islands)
Letter of Sponsorship from the future employment provider (Non-UK/Non-EEA only – valid only for applicants
residing outside of the UK at time of application)
Letter from the Head Teacher or College Principal (16/19 year olds in full time education) – only used in exceptional
circumstances when all other documents have been exhausted) must be UK only.
2. Right to Work List of Acceptable Documents
In order to undertake this essential pre-employment check, you must provide one of the documents or combinations of
documents in List A or List B below as proof that you are allowed to work in the UK.
Please note, we can only accept original documents, unless told otherwise in your offer letter.
Acceptable Documents List A
These documents show that the holder is not subject to immigration control, or has no restrictions on their stay, so
they have an ongoing right to work in the UK.

A passport showing the holder is a British citizen or a citizen of the UK and colonies having the right of abode in
the UK.

A passport or national identity card showing that the holder is a national of an EEA country or Switzerland.

A residence permit, registration certificate or document certifying or indicating permanent residence issued by
the Home Office to a national of an EEA country or Switzerland.

A permanent residence card issued by the Home Office to a family member of a national of an EEA country or
Switzerland.

A Biometric Residence Permit issued by the Home Office to the holder indicating that they are allowed to stay
indefinitely in the UK or have no time limit on their stay in the UK.

A passport or other travel document endorsed to show that the holder is exempt from immigration control, is
allowed to stay indefinitely in the UK, has the right of abode in the UK, or have no time limit on their stay in the
UK.
If you do not have one of the above documents, one of the documents below can be accepted provided that it is
produced in combination with an official document issued by a government agency or a previous employer giving
your name and permanent national insurance number.

An Immigration Status Document issued by the Home Office to the holder with an endorsement indicating that
the person named in it is allowed to stay indefinitely in the UK or has no time limit on their stay in the UK.

A full birth or adoption certificate issued in the UK stating the name(s) of at least one of the holder’s parents.

A birth or adoption certificate issued in the Channel Islands, the Isle of Man or Ireland.

Certificate of registration or naturalisation as a British Citizen.

A letter issued to the holder by the Home Office indicating that the person is allowed to stay indefinitely in the
UK.
If you do not have any of the above documents, we can accept one of the documents from the list below.
Acceptable Documents List B
These documents show that the holder has been granted leave or to enter or to remain in the UK from a limited
period of time and, or, has restrictions on their right to work.

A passport or travel document endorsed to show that the holder is allowed to stay in the UK and is allowed to
do the work in question.

A Biometric Residence Permit issued by the Home Office to the holder indicating that they can stay in the UK
and are allowed to do the work offered.

A residence card or document issued by the Home Office to a family member of a national of an EEA country or
Switzerland.

A work permit or other approval to work issued by the Home Office together with either a passport or travel
document showing the holder is allowed to stay in the UK and to do work or a letter issued by the Home Office
confirming the same.

A Certificate of Application less than 6 months old issued by the Home Office to or for the family member of a
national of an EEA country or Switzerland stating that they are allowed to work together with a positive
verification letter from the Home Office’s Employer Checking Service.

An Application Registration Card issued by the Home Office indicating that they are allowed to work, together
with a positive verification letter from the Home Office’s Employer Checking Service.

An Immigration Status Document issued by the Home Office indicating that they can stay in the UK and are
allowed to do the type of work together with an official document issued by a previous employer or
Government agency with the name and National Insurance number.
