Performance Standard
ICT-TVL:
MATH: The learner demonstrates understanding of Math: Create models of plane figures and formulate and
key concepts of geometry of shapes and sizes, and solve accurately authentic problems involving sides and
geometric relationships.

risk reduction
STS - The learners demonstrate understanding of
fundamental physical quantities in science and the
importance of accuracy in measurement.
STS - The learner should be able to use math language
in expressing ideas in physics and chemistry.
STM - Demonstrates understanding of key
concepts of algebraic expressions, the properties
of real numbers as applied in linear equations in
one variable.
STM - Creates situations using written, and algebraic
methods in solving problems involving linear equations
in one variable
Overview of the Assessment Activity (Provide a clear and concise description of your activity)
The learners are tasked to create a road map of their community through useful applications.
The map will be used to identify at least one accident-prone area in their neighborhood and suggest road
safety measures based on their understanding of the motion of objects. In your road map, plot at least 5 points
that will show some establishments, landmarks or location points. Name these points as point A - E. Lists the
distances of each point from one another, and use these informations to write 5 word problems under SLM 3.2.
Assessment Method/Methods (Put an X Mark on the blank where appropriate)

Assessment Activity
_______Tests
_______Talking to Learners
You are given the freedom to choose one role and do the corresponding product/performance.
Goal – Your task is to create a road map of their community using any useful application.
. Identify an accident-prone area and suggest road safety measures based on your understanding of the
motion of objects.
Role – You are:
A. an aspiring engineer
Audience – Your family, friends, neighbors, and teachers.
Situation – As the government lowered the Covid Alert Level in your community, more people are excited to
go out of their houses. This means that more people and vehicles are moving on the road which may cause
accidents. You want to help your community in preventing road accidents by identifying an accident-prone area
in your neighborhood and suggesting safety measures.
Product/Performance
A. As an aspiring engineer, you will create a road map of your community.
In creating the road map, it is important to consider the following:
1. (Flow chart) -TLE
2. Draw the map using geometric figures (point, line, and plane). -MATHEMATICS
3. Indicate the distance and displacement line from your home (initial position) to the
accident-prone area (final position) in your neighborhood. Write a short explanation of why
road accidents occur based on your understanding of the motion (speed and velocity) of
objects and suggest at least 3 ways or precautionary measures to prevent road accidents. –
SCIENCE
4. STS - Convert the 5 measured distances from your 5 chosen location from Kilometers to
Meters.
5. STM - From the Road MAP, choose 5 location points and determine their
distances from one another. From the lists of distances, create 5 problems
involving SLM 3.2. Show how you solve the 5 problems using representation /
solving methods.
Standards- Your output must meet the standards found in the given criteria.
Expected Output:
A. A road map of your community
