A maintenance plan helps to make sure your vessel (and its equipment) is safe, fit for its
intended use and operating limits, is sound and serviceable and complies with the
maritime and marine protection rules.
This template (and guidance information) will help you to develop and write your own maintenance plan
(or plans), in a form that is acceptable to the Director of Maritime New Zealand (MNZ).
You need to include your maintenance plan when you send your application for a Maritime Transport
Operator Certificate (MTOC) to Maritime New Zealand. The maintenance plan is part of your MOSS
(Maritime Operator Safety System) Operator Plan.
About this template
You need one maintenance plan for each vessel in your operation.
This template has six sections:
1. Voyage checks
2. Monthly checks and routine maintenance
3. Annual checks and routine maintenance
4. Maintenance log
5. Record of maintenance plan amendments
6. Maintenance providers and shore-side contractors.

The checks you do before and after each voyage and while at sea, depend on your vessel and operation.

2. Monthly checks and routine
maintenance
List the items that need to be checked and maintained on a monthly basis, over at least
the next five years.
Each month when you check an item, add your initials and the date in the appropriate box. You also
need to indicate whether the item is:
 Okay () This indicates the item has been successfully checked or maintained and no further action is required.
 Further action is required (X) The details of which need to be recorded in the maintenance log.
 Check not required (N/A).
If the box for the current month is blank, then the item still needs checking.
To make your table cover at least the next five years – prepare your table for the first year, copy it four
or more times and then update the year on each copy.
The table below gives examples of items that may need checks and maintenance on a monthly
basis. Update this table with the items that are appropriate for your vessel. Add more rows where
you need them.

Record the annual checks and routine maintenance that is required for the next five years
for your vessel.
Just as for the monthly checks, indicate whether each item is:
 Okay () This indicates the item has been successfully checked or maintained and no further action is required.
 Further action is required (X) The details of which need to be recorded in the maintenance log.
 Check not required (N/A).
Remember to put your initials and the date in the ‘date checked’ box. If the ‘date checked’ box is blank,
then you still need to check that item.
