ALLIANCE OF ABILITIES SANTA ANA COLLEGE OTA FACULTY-LED FIELDWORK OTA LEVEL II FIELDWORK OBJECTIVES WEEK 1 – BY THE END OF WEEK 1 STUDENT SHOULD BE ABLE TO: • Review and sign FWII Site Specific Objectives • Familiarize self with the Clubhouse International Model and Clubhouse International Standards • Adhere to all Alliance of Abilities policies and procedures • Familiarize self with staff names and responsibilities (see organizational chart in google drive) • Familiarize self with daily routine of the clubhouse (on site or virtual) • Develop rapport with clubhouse members and staff • Become familiar with Google Drive, Resources, Google Slides, SlidesGo • Become familiar with Dropbox (SOAP Notes and reports) • Become familiar with SLACK • Become familiar with Zoom • Develop plans to lead groups for WEEK 2 • Schedule meeting with OTS meeting for training and transfer of care • Educational Discussion w/FWEd – Tips for Virtual Group Treatment & Cole’s 7 Steps WEEK 2 -- BY THE END OF WEEK 2 STUDENT SHOULD BE ABLE TO: • Plan and co-lead groups and complete documentation on these groups o Analyze activity and breakdown in appropriate steps o Adapt activities to meet specific needs of different club members o Be prepared to lead these one hour long groups o Group activities should be occupation-based • Identify the various parts of a SOAP note (group and individual) and use appropriately • Accurately report clinical observations of assigned patient's progress to FWEd • Share documentation with appropriate Alliance of Abilities lead staff • Orient staff and club members as to the purposes and functions of OT when you introduce yourself as the new OTAS leading the group • Be assertive (but polite and respectful) with those who interrupt group activities • Define problem areas for possible future treatment planning using information from observations of club members during groups • You may be co-treating individual club members with OT students • Educational Discussion w/FWEd – SOAP Notes WEEK 3 -- BY THE END OF WEEK 3 STUDENT SHOULD BE ABLE TO: • Lead a full caseload of groups with minimal to moderate supervision from FWEd • Recognize and respond to non-verbal communication or concerns about club house members (in person and remote and consider safety issues as well as emotional/physical) • Confer with FWEd and unit leaders regarding club member's progress and/or concerns • Show ability to adapt activities to meet group club member needs • Encourage and allow club members to grow in independence (leading groups or individual sessions) • Write daily SOAP notes (group and individual) with supervision • Starts to demonstrate effective time management with some input from FWEd • Weekly Assignment - Arrange to meet with one club member for an Occupational Profile Interview • Educational Discussion w/FWEd - Review OTPF (4th edition) and Occupational Profile WEEK 4 -- BY THE END OF WEEK 4 STUDENT SHOULD BE ABLE TO: • Demonstrates initiation of researching evidence-based practice (EBP) for effective treatment in Clubhouse or Telehealth • Assess effectiveness of treatment program, make appropriate changes, and inform club member of the need for changes • Write daily notes with supervision and submits to appropriate staff • Continues to lead groups and supervise/train FWI students or OT/OTA students • Demonstrates effective time management with little input from FWEd • Arrange a time to review Midterm FWPE with FWEd • Week 4 Assignment - Complete Occupational Profile Interview and identify two occupation-based goals with the member • Educational Discussion w/FWEd - Writing COAST Goals WEEK 5 – BY THE END OF WEEK 5 STUDENT SHOULD BE ABLE TO: • Continues to lead groups with minimal input from FWEd • Write daily notes with supervision and submits to appropriate staff • Demonstrates effective time management with independence • Continues to demonstrate competency with assessments and treatment planning with individual treatment sessions • Week 5 Assignment - Submit Occupational Profile with two occupation-based goals to FWEd • Educational Discussion w/FWEd- Review Occupational Profile WEEK 6 • Continue to collaborate with OT/OTS on treatment goals, treatment planning, and assessments • Continue to observe and contribute to assessment with OT/OTS • Continue assessments with 2nd club member who has needs that are occupation-based and does not already have a comprehensive OT report OR one that already has a comprehensive report, but has made progress so the report needs to be updated • Week 6 Assignment - Submit EBP article for effective treatment in Clubhouses or Telehealth • Educational Discussion w/FWEd- Review and discuss EBP WEEK 7 • Introduce and orient new OTA students to groups you are running • Coordinate meetings(s) with OTA student and have student run half the groups the following week. They will lead the group but with your support • Continue to observe and collaborate on assessments, treatment planning, and working with club members who need individual treatment with OT/OTAS • Start mentioning on the Virtual Clubhouse that next week will be your last week and you’ll be running some special activities on your last day • Week 7 Assignment – Schedule meetings with OTA students • Educational Discussion w/FWEd- Orient OTA students (see Week 1) WEEK 8 • Arrange a time to go over the FWPE and prepare SEFWE to share with FWEd • Continue to meet with OTA students for debriefs to help brainstorm on how to run groups effectively • Continue to focus on assessment, treatment planning, and working with club members who need individual treatment • • • • Prepare for the last day of FWII – you’ll lead activities during Fun Friday and give awards to those club members you want to recognize for the effort they put forth to progress towards their OT goals Final debrief – be sure the OTA students have the information they need to continue leading groups and individual treatments Week 8 Assignment – Plan activities for Final Fun Friday and provide awards to 2-3 members Educational Discussion w/FWEd- Discuss any additional recommendations for OTA students _________________________________ Fieldwork Educator (Print Name) _______________________________ FWII Student (Print Name) _________________________________ FWEd signature & credentials _______________________________ FWII Student signature _________________________________ Date ________________________________ Date
