FOR PATIENTS PRESENTING WITH HEART FAILURE EXACERBATION/VOLUME OVERLOAD
LOW Risk HF patient
HFpEF or HFrEF with:
 SPB >100 (≤5pts from baseline)
 HR <100
 O2 sat >90%
 Creatinine <2.0 (or at baseline)
Disposition:
 IV Lasix bolus
 Discharge with referral to IV
Diuresis Clinic
MODERATE Risk HF patient
HFpEF or HFrEF with:
 SBP >100 (≤10pts from baseline)
 HR <110
 O2 sat >90%, or >90% on ≤2L/min
O2
 Creatinine <2.0 (or near baseline)
Disposition:
 Admit to Observation Unit for
diuresis
 Discharge with referral to IV
Diuresis Clinic
HIGH Risk HF patient
HFpEF or HFrEF with:
 O2 sat <90%, or >90% on >2L/min
O2
 +/- Respiratory distress
 Creatinine >2.0
 Lactate elevated
 Need for inotrope or vasopressor
Disposition:
 Admit to inpatient
IV DIURESIS CLINIC REFERRAL
