79 years old male patient with history o DM , HTN , CKD wih
basline Cr 2.5 .
presnted to our ER on 13/1/2019 complaining of shortness of breath ,
hemoptysis , chest discomfort
vitals : BP 125/77 HR 88 o2 sat 90 on room ir improved with o face
mask 5 L /in .
on exam : diffuse ung crepitations
no heart murmurs
+ 1 LL edema
no bleeding
labs : Cr 6 BUN 144 Hb 11 PLT 205 CRP 350
urine analysis : + 3 blood , + 1 protien
CXR : bilateral lng opaciy suggestive of pulmonary hge ?
Impresion : pulmonary renal syndrome ( RPGN ) the patient need urgent
ICU admission with nehrology managment an possible plasmapharesis .
