1) Present Level of Performance:
a. Student loves to talk about his hobbies and activities when he is prompted.
However, when the conversation is “boring,” he will provide close-ended
statements “yes,” “no,” “I do not know,” and will not follow up with a statement
unless he is prompted. He also speeds through his assignments as he “wants to get
them over with.” He needs to continue to work on taking his time through
assignments while also paying attention in class.
2) Current Annual Goal:
a. In one year, the student will be able to identify and discuss the consequences of
rushing through school assignments. At the same time, he should also explore
coping strategies he might experience when he becomes frustrated or
overwhelmed in class. He will utilize learned strategies and coping skills to
increase productivity, insight, and achievement in this period.
3) Progress in meeting goals:
a. He struggles with pacing himself throughout the examinations, and currently, he
is not sure if he does not understand the material or is rushing because of
disinterest. When asked about this, he answers with closed-ended statements.
He also lacks interest in school and is easily distracted by activities on his
computer (google chat etc.)
4) Recommended including frequency
a. Student will continue to benefit from one-to-one counseling 1x week for 30
minutes
5) New Annual goals
a. Student will continue to focus on becoming aware of the reason for his disinterest
in school. Achievement of this goal will be demonstrated by the student freely
discussing his motivation/disinterest in sessions in three out of four consecutive
weeks.
b. With awareness of the consequences of rapidly completing an examination (Low
grades), student will utilize taught strategies to slow down while taking exams,
and his grades will improve. This will be demonstrated with weekly self-reports
grades by student during counseling sessions and improved exam grades (exam
grades will improve by an average of 5% per semester)
c. Student will utilize taught coping skills to increase his frustration tolerance in the
classroom. This will be demonstrated by student self-reports and classroom
teacher observations of zero maladaptive behaviors stemming from frustration
(crying, yelling, leaving the classroom, etc.) over three consecutive weeks.
