WHAT HAPPENED? (Brief factual description of the incident)
Bus 1104 (KH-030) service went to Ibn Zahr, Ibn Sina and Sharq to drop and pick up the employees for work,
While on our way on the road going home, there are abnormal things noticeable from the bus. The whole bus wiggles the driver said
that there is a problem regarding the tires and the smell of diesel leaking profusely at the front engine that causes strong smell inside
the bus. As for the drivers information he indicated that the leakage happens three days ago.
The leak might cause fire due to heat and the tires was not been inspected and might cause an accident, at this rate this will cause
another accident the same what happen previously on the equipment.
Near miss due to diesel leakage, property loss on diesel leaking profusely for three days and damage of the tires has occurred.

IMMEDIATE ACTIONS TAKEN
Informed the supervisor about the situation of the vehicle.
Advised the driver not to use the vehicle for service until it’s already repaired.
ROOT CAUSE

Poor service maintenance for the vehicle.

No visual inspection as per use of the vehicle.

Different drivers using the vehicle.

Driver never care for the vehicle.

Logistics still allow the driver to use the vehicle even it have a problem already.
RECOMMENDATIONS:

Regular maintenance and inspections of the vehicles: tires, oil, hydraulics and so on.

Evaluation of the drivers who are using the vehicles.

Immediately report any abnormalities of the equipment to the concerned person.

Convey the message to all Khidmah drivers and operators to have awareness regarding on this matter. A lesson
learned for all and to prevent occurrence.
