AFFIDAVIT OF DISCREPANCY
I, LEMUEL VICTOR DALUMPINES SAMONTAN, of legal age, Filipino, married to
ARITHA BIANCA PINGA SAMONTAN and a resident of No. 62 Zenith Street South Meridian
Homes , Brgy. Salitran 4, Dasmarinas City, Province of Cavite, after having been sworn to
according to law hereby depose and say:
1. That I am the son of the spouses Renie Dagandan Samontan and Marilyn Pinaflor
Dalumpines born on August 22, 1989;
2. That in my Marriage Certificate which I obtained during my marriage to ARITHA
BIANCA PINGA SAMONTAN dated March 24, 2014 with registry no. 2014-415, my
middle name is spelled as “DALUPINES” instead of "DALUMPINES" to which I have
been used to and as what is indicated in my school records, birth certificate, passport, and
other public documents;
3. That the reason why my middles name was spelled as “DALUPINES” instead of
“DALUMPINES” was due to typographical error made by the person who prepared the
document;
4. That the names LEMUEL VICTOR DALUMPINES SAMONTAN and LEMUEL
VICTOR DALUPINES SAMONTAN belong to one and the same person, the
undersigned herein;
5. That I am executing this affidavit to attest to the truth of foregoing circumstances and for
the purpose of setting the records straight and declaring that LEMUEL VICTOR
DALUMPINES SAMONTAN and LEMUEL VICTOR DALUPINES SAMONTAN
refer to one and the same person.
