SUBJECT: SURVEY QUESTIONNAIRE ON BENEFICIAL OWNERSHIP
The SECURITIES AND EXCHANGE COMMISSION through the Anti-Money Laundering
Division of the Enforcement and Investor Protection Department (EIPD-AMLD) is currently
enhancing its outreach program and information dissemination campaign on beneficial
ownership.
For this purpose, the EIPD-AMLD is conducting an assessment on the stakeholders’
level of awareness and understanding of the concept of beneficial ownership and the
requirement to disclose beneficial ownership information in the General Information Sheet
(GIS).
All Corporate Secretaries, Directors/Trustees and Officers of SEC registered
corporations are enjoined to accomplish this Survey Questionnaire. The questionnaire is for
information gathering purposes only and does not constitute a review of any corporation’s
performance. The responses will be kept confidential and will not be shared with other
institutions and stakeholders.
The Survey Questionnaire must be accomplished and submitted online on or before
