GENERAL DESCRIPTION:
A committee led by an appointed chair to collect data of those persons or organizations
giving noteworthy service to the profession of occupational therapy in Florida and to
distribute awards to those persons. Awards will include the following:

DUTIES AND RESPONSIBILITIES:
1. Committee Formation:
a. The President upon recommendation of the Member Concerns
Coordinator shall appoint the chair from the FOTA membership. The chair
will be a member of FOTA in good standing. The term of the chair shall
be for two years coinciding with the term of the FOTA President.
b. Committee members (3) shall be appointed by the chair as needed. The
chair shall determine their term.
1. Duties of the committee members include, but are not limited to:
a. Identify persons or organizations in the committee members’ local
area for FOTA awards.
b. Follow-through on nominations for FOTA awards (i.e., collecting
data) and
c. Assist with awards presentation at the FOTA Annual Conference.
2. Data Collection for the FOTA Awards:
a. Nominations forms shall be distributed throughout the fiscal year to
anyone desiring to make a nomination for an award. Nomination forms
will be available as a PDF document on the website www.flota.org. (See
sample 1). The chair shall collect the completed nomination forms and all
necessary supporting documentation. A deadline for submitting all
nominations will be decided upon by the chair. The deadline will be no

later than two months prior to the FOTA Annual Conference/Meeting.
Any revisions to the nomination forms shall be the responsibility of the
chair.
Award recipients will be notified by a letter (see sample 2) inviting them
to the Awards Presentation. The FOTA Vice-President and/ or Conference
Convener must be notified of award recipients attending the awards
presentation for registration purposes.
The Chair and/or designee will complete a list of Award winners for the
FOTA Conference handbook and forward this to the Vice-President and/or
Conference Convener no later than 4 weeks prior to the event.
Obtain a quote for plaques and submit this to the Member Concerns
Coordinator. A FOTA Purchase requisition form needs to be completed
and sent to the FOTA Treasurer for approval. Order plaques from a local
store.
Presentation and distribution of the awards will be at the FOTA Annual
Conference/ Meeting. In the event an award recipient or a representative
for them is unable to attend the presentation, arrangements must be made
to have the plaque mailed to the recipient.
Arrangements for the Awards Presentation will be coordinated with the
Member Concerns Coordinator and the Vice President/ Conference
Convener.
Award Descriptions and Qualifications:
a. The Louise Samson Leadership Award:
This award is in recognition of leadership that has contributed to the
recognition and advancement of occupational therapy in Florida.
Winners can be occupational therapists and/or occupational therapy
assistants. FOTA will award one award each year in this category.
The recipient will receive a plaque.
The recipient will be profiled on the website and also in the Focus
newsletter.
b. The David D Clark Award of Excellence:
This award is for longstanding and significant contribution to the
profession of occupational therapy and FOTA in Florida.
Recipients can be occupational therapists and/or occupational therapy
assistants and must be FOTA members in good standing.
Recipients will receive a plaque.
Recipients will be profiled on the website and also in the Focus newsletter.
c. FOTA Award of Recognition:
This award is for outstanding contribution to the recognition and
advancement of occupational therapy in Florida.
There can be more than one recipient for this award and can be
occupational therapists and /or occupational therapy assistants.
Recipients will receive a pin and a certificate.
d. FOTA Legislator of the Year:
This award is for significant contribution to the recognition and
advancement of occupational therapy in Florida by a state legislator.
The recipient will receive a framed certificate and be profiled on the
website and in the Focus newsletter.
e. FOTA Award of Appreciation:
This award is to other persons and/or organizations that have rendered
special services to FOTA or the occupational therapy profession in
Florida.
There can be more than one recipient for this award. Recipients cannot be
occupational therapy practitioners.
Recipients will receive a framed certificate.
f. Myra McDaniel’s Student Scholarship Fund:
a. Collect necessary documents from the grant applicant. The
application form will be available as a PDF document on the
website www.flota.org.
b. The Committee will review the application, per grant guidelines
(see sample 3 – Policy on Administration of Myra McDaniel’s
Student ScholarshipFund).
c. The Committee will notify the applicant and the Executive Board
of its decision.
d. The Chair will request a check from the FOTA Treasurer to be sent
to grant applicant, if approved.
e. The Chair will compile a list twice a year and forward a list of
grant applicants with contact information to the Vice-President
and/or Conference Convener, so that they have a pool of eligible
volunteers for the FOTA Annual Conference.
g. Lela Llorens Research Fund:
This program has been suspended by FOTA. The Chair in conjunction
with the Executive Board will complete a review of the guidelines for its
administration.
4. Timelines:
a. Prepare a report when requested by the President. These need to be
submitted to both the Secretary and Member Concerns Coordinator within
the required timeline. The Chair can attend all Board meetings.
b. Attend Board meetings to present any committee action items or
recommendations. If unable to attend, the Member Concerns Coordinator
will present action items or recommendations.
c. Present an annual committee report at the FOTA Membership meeting. If
you are unable to attend, the Member Concerns Coordinator will present
your report.
d. Submit a proposed budget to the Treasurer and Member Concerns
Coordinator no later than April 15 of each year.
MEETINGS
1. FOTA Executive Board Meetings. If you are unable to attend, the Member
Concerns Coordinator will present your recommendations or action items.
2. FOTA Annual Membership Meeting If you are unable to attend, the Member
Concerns Coordinator will present the report.
