There are THREE parts to this form and you must complete all three in order to move forward in
this course: A) a goal setting exercise, B) a form requesting permission to use samples of your
work for instructional purposes, and C) a plagiarism/education acknowledgment.
GOAL SETTING
This is your education. To make the most of it, set 3 goals for yourself this class and type them
in below. At the end of this document, please type your name and date confirming that you have
read the college plagiarism policy.

PERMISSION TO USE STUDENT WORK
This section of the form is to request permission to present and reproduce samples of work
produced in this class for instructional and research purposes at Lansing Community College.
This means that the instructor may want to use samples of your work to help other students learn
about effective writing techniques. This also means that your instructor, as a way to help
advance knowledge about effective teaching practices, might refer to your work in a scholarly
presentation or publication. Please indicate your permission/ desire to have samples used or not

UNDERSTANDING PLAGIARISM
It is extremely important that every student understands what constitutes plagiarism, and upholds
the standards of academic integrity. Plagiarism is not just the blatant use of another student’s
paper. It can also occur when using sources improperly. Please read the following statements
regarding plagiarism, and then electronically sign below. By doing so, you have agreed that
you understand what plagiarism is and you accept that any evidence of plagiarism in your
work may result in a zero for the assignment and possible disciplinary action (for more
information on what constitutes plagiarism and also its consequences, see the LCC policy
in the Syllabus).
What is Plagiarism?

Using too many ideas gathered through research and not enough of your own

Cutting and pasting information from sources without in-text citations (also
known as parenthetical citations) and/or quotation marks

Using examples, statistics, facts, etc. without in-text citations

Copying word-for-word THREE or more words in a row from a source with no
quotation marks and/or in-text citations

“Borrowing” an essay from someone else or off of the internet; “giving” someone
a paper; buying a paper; “revising” someone else’s paper

Cutting and pasting whole paragraphs, etc. from outside sources (even with
parenthetical citations and quotation marks)

Moving a few words around and calling them your own (with or without a
citation)

Reusing your own paper created for another project or course

Write your own ideas first; properly incorporate research that supports these ideas

Use in-text citations and quotations marks where needed; make sure that you have
a reference page. Cite ALL sources and research material
Ask for help if you are not clear on any rules for citing sources
