AFFIDAVIT OF UNDERTAKING
I, GERALD BAUTISTA HENANIBAN, of legal age, single/married, Filipino and a resident
of P-ALMASIGA, PANGABUGAN, BUTUAN CITY after having been duly sworn to in
accordance with the law, do hereby depose and say THAT;
1. That I am an applicant of Jail Officer 1 of the Bureau of Jail Management and
Penology Caraga;
2. That I am scheduled to undergo neuropsychiatric interview through tele- interview
using my personal Facebook account between me and that of my interviewer on
April
2021;
3. That due to the privacy and confidentiality required by ethical standard in the
practice of profession by the interviewer, I prohibit myself to any video and audio
recording, picture taking or any way that the interview be heard by the person/s:
4. That any item taken from the above- cited circumstances be more aggravating if
these are reproduced, shared, talked- with or any possible means that any content
of the interview is leaked;
5. I fully understand that any violation of this undertaking would render me criminally,
civilly and administratively liable pursuant to Republic Act No. 4200 as amended
or otherwise known as the Anti- Wiretapping Law, and other pertinent laws; and
6. I, GERALD BAUTISTA HENANIBAN, assured and take full responsibility on the
observance of these undertaking.
