Crisis Scenario 1 – Pre-filter Separation Step 1: Arterial pump off and CPG pump off if running Comm: Line separation, we are off bypass at (note time). Please clamp the lines at the table. Step 2: Isolate patient - Clamp arterial line and move clamp on recirc line to venous line. Comm: I need 2 mins, I will let you know when I’m ready to go back on. Step 3: Reconnect the separated line. - If reservoir volume is low, drop prime sufficient to replace lost volume (estimate) Step 4: Stop bypass timer and start an ischemic timer. Step 5: Begin recirculating. Comm: Is the arterial line obstructed at the field? Step 6: Follow the circuit and confirm other connections are secure. Step 7: Completely de-air the circuit. - Check ALF, bypass line, oxygenator, CP line, and boot for trapped air - If needed, re-prime the red box transducer and re-zero. Comm: I am ready to go back on bypass, all clamps off at the field? Step 8: Check line placement Step 9: Reinitiate bypass. Comm: Back up to full flow at (note time), ischemic time was _____mins. Step 10: Restart bypass timer. Step 11: Clean up Comm: Circuit was contaminated, additional antibiotics may be needed. Comm: I’ll draw another set of labs. Make a note of additional prime used.
