BLOOD SAMPLE REJECTION CRITERIA Code: Version: Effective from: AHMC-LAB-PRO:07 V1 01-NOV-2019 Review interval: Pages: Signature authorizer: 1 year Page 3 of 4 GENERAL INFORMATION Purpose: To provide a standard procedure for rejecting unacceptable specimens and hence improve, or reduce, the number of medical errors in overall patient care. Scope: Area – Laboratory & Clinical Areas of AlHayat Medical Center Staff – Nurses, Phlebotomist & Technologists Definitions: Blood sample rejection criteria: a method in which the specimen is rejected for the specified observation/result/analysis by the Laboratory. POLICY The following criteria will be used for specimen rejection: Unlabeled, mislabelled, or incompletely labeled specimens will be discarded by the Laboratory and a new collection will be requested immediately. PROCEDURE All samples must be labeled at the patient’s bedside collection and within sight of the patient. Barcoded lab specimen labels are generated with the Laboratory test order. The team will use at least two patient identifiers before providing Laboratory services. Discrepancy between the patient identification on the test request, and on the specimen label, will cause the specimen to be discarded by the Laboratory and a new collection to be requested. Incomplete data on the request form will cause delay in the tests processing and inconvenient results. The request should be fulfilled with the patient’s full name, requesting physician’s name signature & stamp, file number, relevant clinical information, gender, date of birth, date and time the primary sample was collected, and date and time when the Laboratory received the sample. Specimen(s) deemed unacceptable due to the requirements of test methodology (i.e. hemolysis, lipemia, short samples, wrong tube, or wrong specimen type). Pre-analytic specimen requirements not met (e.g. fasting hours, stopped antibiotics 3 days before culture and inappropriate volume of blood). Pre-analytic handling and transport requirements not met (e.g. frozen sample, specimen container is broken or leaking). Rejection Notification Protocol o Upon receipt of an unacceptable specimen in the Laboratory, the ordering physician will be notified. If he/she cannot be contacted, the primary nurse taking care of the patient is notified. In the event of a contaminated specimen, Laboratory results of that contaminated specimen will NOT be communicated to the floor, but a new specimen will be requested. The Laboratory personnel will document into the rejected sample log with the name of the person contacted and the reason for rejection.
