Please note that as a subcontractor to Bulletproof Electric, LLC you are governed
by the same mandatory requirements in place for Bulletproof Electric, LLC as
stated for our client Western Midstream (WES). You will be provided with a list of
requirements and must follow both that and any/all OSHA regulations. The
following is a list of all Mandatory Requirement Training items. Employees should
carry or be able to provide certification cards and you may be asked to provide
training sign-in sheets. WE request that your employees join any Tool Talk Health
and Safety Sessions held during the time you are performing work as a
subcontractor to us. Please sign and date this form and return to us along with our
