Learning Log Sign Off Sheet for UCL National Award for SEN Co-ordination PG Cert
The Learning Log Sheet provided to course participants is an essential part of the NASENCo final
qualification as it ensures that practice-based learning outcomes are achieved and evidenced by
course participants. Course participants must:
1. Ensure that each outcome on the log has been signed by the Learning Mentor or other
relevant person.
2. Maintain a record of evidence to underpin each learning outcome (e.g. example of
presentation given as part of an INSET day or record of a Person-Centred Planning
meeting). This evidence must be held by the school for one calendar year after the
qualification has been awarded (31st October following the course end).
3. Ensure that the Learning Mentor and Head Teacher/SEN Governor have signed the
portion below.
4. Upload this signed sheet to Moodle on or before the hand-in date of the Module 2
Assignment.
