Identify and avoid the factors that causes hypertension. Identify preventive measures and management when hypertension occurs. To encourage the family to continue taking presumptive measures. INTERVENTION Broaden the knowledge of the family about hypertension. Discuss the effect of hypertension on the body, preventive measures, proper management and treatments. Discuss the factors causing hypertension and its signs and symptoms. Discuss the complications of hypertension in the body and daily living. Encourage a positive attitude. EVALUATION Goal Met, as the family can be able to identify the problem and its condition, and took necessary action to prevent and manage when hypertension occurs.
