CHAPTER 1
GENERAL
1-1. PURPOSE: This SOP establishes standards, policies and procedures for the
selection, training, and licensing of personnel to become Army motor vehicle drivers
and operators of powered or self-propelled equipment. When used with the 372nd QM
BN (PS) Drivers Licensing Guide and AR 600-55 it constitutes the Licensing Program
for the 372nd QM BN (PS).
1-2. APPLICABILITY: This SOP applies to all units and activities of the 372nd QM BN
(PS). Individuals operating a vehicle and/ mechanical equipment for the 372nd QM BN
(PS) must possess a valid Operators Permit ID or DA Form 5984-E (ARMY
STANDARD PERMIT) and a valid State Issued Driver’s License.
1-3. REFERENCES: This 372nd QM BN (PS) Standard Operating Procedure
supplements AR 600-55 dated 17 September 2019. Required and related publications
are listed in Appendix A.
1-4. DELEGATION OF AUTHORITY: Drivers Training Program is a battalion level
program. This SOP delegates subordinate Commanders down to company /
detachment level the authority to train, test, and license vehicle and equipment
operators IAW this SOP and AR 600-55.

CHAPTER 2
RESPONSIBILITIES
2-1. RESPONSIBILITIES: The following are the basis for implementation of the 372nd
QM BN (PS) Licensing Program and restate, for emphasis, selected responsibilities
from AR 600-55 (See AR 600-55, CH 1-4 for a complete list of duties and
responsibilities).
2-2. 372nd QM BN (PS) Headquarters
a. Maintain and update the 372nd QM BN (PS) Drivers Licensing and Dispatching
SOP, 372nd QM BN (PS) Drivers Licensing Guide, and any additional applicable forms
and reference material.
b. Develop and publish a 372nd QM BN (PS) Drivers Licensing Guide (See
Appendix I) to serve as the unit(s) “Step by Step” licensing tool. Chapters 3-7
summarize these licensing steps but Appendix G provides a more detailed explanation
to the guide.
c. Delegate to subordinate commanders in writing the authority to train, test, and
license noncommercial vehicle and equipment operators.
d. Appoint in writing a Master Driver in the rank of SSG or above that is the primary
advisor to the battalion commander and subordinate units for all facets of operator
training.
e. Inspect the licensing program as a component of the Organizational Inspection
Program.
2-3. COMMANDERS:
a. Implement and ensure compliance with AR 600-55, 372nd QM BN (PS) Drivers
Licensing and Dispatching SOP, and guidance provided in the 372nd QM BN (PS)
Drivers Licensing Guide.
b. Appoint a GCSS-Army License Instructor, License Examiner and alternates
(SGT or above). These individuals will administer the unit Driver’s Training Program
working closely with the battalion Master Driver.
c. Ensure training and licensing is conducted IAW this SOP and AR 600-55, to
include maintaining the required individual training/licensing records.
d. License enough individuals to move and operate all unit equipment in the event
of mobilization or deployment.
e. Conduct the Commander's Interview using the interview format in Appendix C or
AR 600-55, Appendix B. This may be delegated in writing to an SSG or above.

f. Ensure operator’s manuals are on hand to meet unit equipment needs.
g. Ensure all vehicles have operators and assistant drivers assigned to them.
2-4. FIRST LINE LEADER/PLATOON LEADER/PLATOON SERGEANT:
a. Ensure that licensed operators are proficient in the use of vehicles/equipment.
Feedback from first line leaders to Instructors/Examiners is an integral part of improving
a licensing program.
b. Verify that licensing training records are maintained for each service member.
c. Maintain a current military license for all assigned squad or section equipment.
d. Ensure all vehicle operators are assigned an assistant driver.
e. Perform quality assurance and compliance with applicable regulations.
2-5. OPERATORS OF MOTOR VEHICLES/TRACKS:
a. Operate vehicles or equipment in a safe and prudent manner.
b. Report unsafe operating conditions of vehicles or equipment.
c. Report all accidents to his or her supervisor and to the motor pool that
dispatched the equipment. Comply with all municipal, State, and military motor vehicle
or equipment regulations.
d. IAW Memorandum from Secretary of the Army Washington, dated 18 APR 2018,
Accident Avoidance Course (AAC) is NO longer required by HQDA.
e. Possess an “ARMY STANDARD” operator permit (DA FORM 5984-E) for the
vehicle/equipment being operated, and possess a valid State Issued Driver’s License.
See example below.
f. Only operate equipment with a qualified assistant driver.

2-6. SENIOR OCCUPANT:
The senior occupant in a military vehicle is the ranking individual present. The senior
occupant will:
a. Ensure the driver is rested and not emotionally or physically impaired from safely
operating the vehicle IAW AR 385-10 CH 11-2, c, (4).
b. Ensure that the seating capacity is not exceeded and restraint systems, when
provided, are worn when the vehicle is in motion. All other Personal Protective
Equipment (PPE) associated with the vehicle will be used as well, i.e. Kevlar/approved
head/ hearing protection, etc.
c. Ensure the driver complies with laws/regulations and operates the vehicle safely
IAW AR 600-55.

SELECTION OF INSTRUCTORS, EXAMINERS, AND OPERATORS
3-1. SELECTION OF INSTRUCTORS AND EXAMINERS
a. The effectiveness of the driver’s training, testing, and licensing program depends
upon the proper selection and training of a unit licensing team. Good operators or
qualified maintenance personnel do not necessarily make good Instructors/Examiners.
Before appointing an individual to be a license instructor or license examiner,
Commanders will consider factors such as technical knowledge, experience, character,
instructor ability, and expertise on unit equipment.
b. All prospective equipment License Instructors / License Examiners will:
(1)
Be SGT/E-5 or above. (Below E-5 provide a Memorandum for Record)
(2)
Be licensed to operate the vehicle or equipment.
(3)
Be appointed in writing for each type of equipment within the unit. An
instructor may be qualified to license operators on more than one type of equipment.
(4)
Have appropriate entries made on personnel qualification records,
evaluation reports, DA Form 348 or 5983-E (Equipment Qualification Record), and DA
Form 5984E as License Instructor / License Examiner. These should not be the same
person, whenever possible.
(5)
Complete Distance Learning Course 920-F92 (DL), “License Instructor /
Examiner” on the Army Learning Management System (ALMS). Must be registered in
Army Training Requirements and Resources System (ATRRS).
3-2. SELECTION OF OPERATORS:
Personnel selected to drive army motor vehicles must possess a valid State issued
driver’s license prior to being tested for a military license. Potential operators not
possessing a State issued license may be issued a ‘ARMY LIMITED” or “ARMY
INCIDENTAL” for special missions but must comply with AR 600-55 license
requirements.
a. Step 1: CURRENT DRIVER’S LICENSE REVIEW: All 372nd QM BN (PS)
vehicle operators must have a valid State issued driver’s license when issued an
“ARMY STANDARD” operator permit. This is to ensure all Soldiers comply with state
law and are able to operate a military vehicle off the installation. This removes the
possibility of issuing a Soldier an Incidental/ Limited use permit restricted for the
installation only, so that the Soldier doesn’t inadvertently drive off the installation.
b. Step 2: COMMANDER'S INTERVIEW: Interviews will be conducted by the
Commander or appointed representative (SSG or above). Areas of concern are
372nd QM BN (PS) DRIVERS LICENSING AND DISPATCHING SOP
maturity, attitude, past driving record, hearing, extreme nervousness, medication, or
any abnormal characteristics or physical restrictions. Some additional factors to
consider are personnel over 25 years of age with driving experience of 1 year or more
during which the applicant has driven 4,000 accident free miles can indicate good
judgment and operation skills. Suggested interview questions are located in AR 600-55,
APPENDIX B-3.
c. Step 3: MEDICAL REVIEW: A medical review must be accomplished IAW AR
600-55, Appendix D, to determine if potential operators for military licensing are
physically and mentally fit to operate government motor vehicles and/or equipment. A
valid Periodic Health Assessment (PHA) can help determine any medical restrictions
that need to be added to the Operator record DA FORM 348 or a disqualifying condition
prior to issue/renewal. A potential operator with any medical restriction/s that would limit
his/her ability to operate vehicles/equipment will report these restrictions to the
instructor, Commander, and supervisor as soon as they are known. The restrictions will
be properly annotated on the Operators Permit ID/DA Form 5984-E and Qualifications
Profile.
d. Step 4: PHYSICAL EVALUATION: Instructors are to ensure the potential
operators possess the minimum physical requirements for safe driving or equipment
operation IAW AR 600-55. Physical Evaluation must be reviewed yearly and renewed
every five years. Information may be obtained from:
(1) A valid State driver’s license.
(2) Instructor observations.
(3) Questioning the applicant on their physical restrictions is an acceptable
substitute for the military physical tests.
(4) Use of the Standard Driver Training and Testing Device, NSN 6930-00-5263639 if available.

CHAPTER 4
OPERATOR TRAINING & TESTING.
4-1. OPERATOR TRAINING:
Training for operators consists of three phases. AR 600-55, Appendix E, provides
guidelines for Commanders to use in establishing operator training programs. Training
Circular (TC) 21-305 series covers Vehicle specific Training Programs. Additional
training material can be found at https://utap.army.mil/Browse.aspx#wheeled.
a. Phase I, Initial Operator Training, must include:
(1)
Course introduction and overview
(2)
State, local, nation, and post laws
(3)
Regulations and responsibilities
(4)
Basic operating procedures
(5)
Emergency procedures
(6)
Accident causes and reports outlined in DA PAM 385-40, Army Accident
Investigations and Reporting, paragraph 4. It is essential that operators be trained to
obtain all vital information at the scene of an accident and to complete all entries on the
Standard Form 91 (SF 91) "Operator's Report of Motor Vehicle Accident" and “DD
FORM 518 accident identification card”. This step will also include a discussion of
operator responsibilities and government liability.
(7)
Hazards and Safety Measures. The intent of the 372nd QM BN (PS)
licensing program is to provide effective training which, when done to standard,
minimizes the likelihood of an accident. Required safety training, to include Risk
Assessment and DD Form 2977, is a key component of this program. The applicant
must receive detailed instruction on the safe operation of equipment to gain an
understanding of risk assessment. This step will also include off road operations,
environmental considerations, convoy operations, and other topics. These steps are
necessary for the safe and proficient operation of vehicles and equipment.

(10) Maintenance, PMCS, and Army Forms. PMCS training provides the
potential operator a vehicle/equipment orientation while also emphasizing operator
maintenance responsibilities. Applicants must demonstrate PMCS proficiency as a
component of the Phase III road test.
(11) Motor Marches and Convoy Operations
b. Phase II, Vehicle-specific Training, consists of hands-on training based on the
operator -10 Technical Manual (TM). Phase II training will be conducted for each piece
of equipment on which the operator is to be licensed. Training will include:
(1)
Introduction to the specific vehicle/equipment
(2)
Written examination.
(3)
Issue Army Learner Permit
(4)
Vehicle operating familiarization using training aids, devices, simulators,
and simulations (when available).

(5)
Operate vehicle/equipment under normal conditions on road driving
courses and in controlled routes during daylight and nighttime using headlights (driving
routes to be approved by local command)
(6)
Operate vehicle off-road using training locations approved by local
command. Focus training content on the unusual conditions defined in operator
technical manual.
(7)
Emergency Situations and vehicle/equipment malfunctions
(8)
Self-recovery methods
(9)
Vehicle/equipment specific capabilities (winch, material handling
equipment, etc.)
c. Phase III, Training Validation and Road Test. Training will include:

4-2. OPERATOR TESTING:
a. Written Examination (Phase I, Initial Operator). The written test will cover the
material covered as outlined in AR 600-55 App. E-3(a), units A through K.
b. Written Examination (Phase II, Vehicle Specific). Phase II training will be
conducted for each vehicle/equipment type on which the operator is to be licensed. The
written test for specific vehicles/equipment must be taken and will consist of a minimum
of 20-25 questions, which are unique to that type of equipment. After the potential
operator takes the test, it will be reviewed with the instructor to ensure a minimum score
of 80% is obtained, correctable to 100%. The open book test will be recorded as part of
the driver’s licensing packet. Unit examiners may create their own tests with
commander approval. However, instructors should use training materials developed by
the TRADOC School proponent for the type of vehicle or equipment on which training is
to be conducted. Written tests are located in the 21-305 series of Training Circulars
found at www.armypubs.army.mil. A Technical manual (TM) can be used for this
training when TC’s are not published.
c. “ARMY LEARNER” Permits.
(1)
Operators will not participate in any hands-on vehicle or equipment
training without a valid Operators Permit ID/DA FORM 5984-E “ARMY LEARNER”
Permit.
(2)
Learner permits for USAR and ARNG will remain valid as long as the
individual is assigned to the unit and is participating in an active driver training program.
Learner permits are valid only during regular training periods (for the period as stated
above) and only under the following conditions:
(a) A qualified driver, equipment instructor, or examiner accompanies the
trainee in the vehicle or on the equipment.

(b) The vehicle or equipment is operated within a controlled driving range (no
other vehicular traffic) or training area under the supervision of a licensed driver or
operator.
(3)
The words "ARMY LEARNER" will be marked on the face of the
Operators Permit ID or DA Form 5984-E. This should be printed from GCSS-Army and
will not require a stamp or overwriting.
d. Road/Performance Test.(Phase III) The purpose of the road/performance test is
to evaluate the driver’s ability to drive safely in most on-the-road situations or to
evaluate the applicant's ability to safely operate equipment. It serves as the basis for
issuing an operator's permit and provides an opportunity for instructional reinforcement
and counseling.
(1)
A checklist for the driver’s road test (Appendix E, "Road Test Score
Sheet", DA FORM 6125) will be used to administer the test. A passing score is 25
errors or less. The hands-on performance test will be given by license examiners to
evaluate operator proficiency on equipment before issuing a license.
(2)
The road test will consist of three scored phases: PMCS test, the vehicle
control test, and the driving test. If the driver fails any phase of the test, the entire road
test will be terminated at that point. (See AR 600-55, Appendix G.)
(3)
Final evaluations will be recorded on DA Form 348/DA Form 5983-E. The
road test or hands-on test results will be included in the individual's documentation
packet and annotated on the Individual Licensing Checklist. Whether the driver passes
or fails, the examiner will review the results of the road test with him or her and bring to
the driver’s attention any weaknesses which require further practice or training.
(4)
Road tests should normally not be given if road or weather conditions
present a hazard. The exception is when testing is specifically for driving under such
conditions.
(5)

Tracked Vehicles. Applicants are required to be tested as specified in TC
(6)
Materials Handling Equipment. Applicants are required to be tested as
specified in TB 600-2.
(7)
Non-Self-Propelled Equipment. Required criteria for licensing applicants
on mechanical or ground support equipment are contained in TB 600-1 and TB 600-2.

CHAPTER 5
LICENSING
5-1. OVERVIEW All training for vehicles and equipment which require licensing under
this SOP (See Appendix D) will be documented on DA Form 348 and the Qualifications
Profile prior to issuing an Operators Permit ID / DA Form 5984-E. DA Form 348 and the
Operator’s Qualification Record will be maintained by the unit / shop on every person
who operates a vehicle or equipment owned or lent by the 372nd QM BN (PS).
Qualification on non-self-propelled equipment will be entered on DA Form 348 and the
Qualifications Profile, Section I, and training will be entered in Section III.
a. Although all units of the 372nd QM BN (PS) are automated under the Global
Combat Support System-Army (GCSS-ARMY), the Qualification Profile is not an
authorized replacement for the DA Form 348. GCSS-ARMY produces a Qualification
Profile that reflects equipment operator’s qualifications that have been locally
transferred into GCSS-ARMY from the DA Form 348. This process was designed to
help streamline automated dispatching procedures using a relational database. The
GCSS-ARMY generated Qualification Profile does not provide the necessary
documentation for the Commander to make an appropriate determination for
authorization of equipment use.
b. The DA Form 348 is a permanent record and the current record will not be
destroyed. All entries will be in black ink or typed unless otherwise stated. See AR 60055 for additional guidance on posting/correcting entries to the DA Form 348.
c. All operator permits will be marked, by GCSS-Army, IAW AR 600-55, Chapter 6.
d. Operator Qualification Records will expire 90 days from the date a new
commander signs assumption of command, five years from issuance, or at the
expiration of the operator’s civilian driver’s license, whichever is first.
5-2. INDIVIDUAL TRAINING RECORDS (See Appendix F Sample Documentation and
Training Folder). Individual Training Records will be kept on each operator and will
consist of:
a. Copy of State Driver’s License (mandatory).
b. DA Form 348 and DA Form 348-1-R (mandatory).
c. Copy of Commanders interview (mandatory).
d. Copy of all hazmat written tests indicating the Test scores (mandatory as
applicable).
e. Copy of Final NVD written test (If Applicable).
f. Copy of Initial NVD Road Performance Test / replaced by re-certification road
test as recertified bi-annually (If Applicable).

g. Copy of DA Form 6125 Bi-Annual check ride road test score sheet for wheeled
equipment or memorandum for record for ground support equipment (mandatory).
5-3. ISSUING THE OPERATOR'S PERMIT:
a. Operators Permit ID or DA Form 5984-E will be issued by a representative of the
Commander, who is appointed in writing, to all personnel who complete the prescribed
training and tests of this SOP and AR 600-55. Whenever there is any reasonable doubt
regarding the authenticity or validity of a potential operators State issued driver’s
license, or suspicion of a poor driving record, a prompt and thorough inquiry must be
conducted before issuing / renewal of the Operators Permit ID / DA Form 5984-E.
b. Standard Permits.
(1)
The Operators Permit ID/DA Form 5984-E standard permit applies to all
vehicle drivers / equipment operators. The words "ARMY STANDARD" will be marked,
by GCSS-Army, on the permit IAW AR 600-55, Chapter 6.
(2)
Permits will be valid while the service member is assigned to the unit. If a
unit member transfers; the gaining unit will verify training and transpose the Soldier’s
information to a new Operators Permit ID /DA Form 5984-E.
(3)
The expiration date of an Operators Permit ID or DA Form 5984-E will be
Set IAW AR 600-55, Chapter 6, paragraph, 6-2. A copy of the license will be maintained
in the operator’s training record.
(4)
Operator Permit Ledger. Each issuing authority will maintain an
automated or manual ledger of all permits issued. The ledger will be organized by date
sequence and will include date of issue, expiration date, permit number, type of
equipment qualified to operate, issuing official and remarks block.
c. All newly assigned operators to the unit will be instructed in local regulations and
procedures before operating vehicles/equipment.
d. Restrictions:
(1)
Mark or stamp on the operator's identification card (DA Form 5984E or
Operators Permit ID) any restrictions such as "with glasses" or "automatic
transmissions only".
(2)
Ensure GCSS-Army marks on the operator's identification card, “ARMY
STANDARD”,” ARMY LEARNER”,” ARMY INCIDENTAL”, or “ARMY LIMITED” for
type of permit being issued.
e. Revocations / Suspensions.

(1)
If an individual's State driver’s license is revoked / suspended, their
military operator permit is not valid and will also be revoked / suspended for the same
suspension period.
(2)
For manual procedures, annotate on the DA Form 348 that the license is
suspended. Retrieve the DA Form 5984-E from the operator and attach it to the
operator's DA Form 348.

CHAPTER 6
SUSTAINMENT TRAINING & TESTING
6-1 PROGRAM Commanders will develop and implement a sustainment-training
program to be conducted IAW AR 600-55, chapter 4-5, at minimum every two years for
any driver with a valid Operators Permit ID /DA Form 5984-E. It will consist of a check
ride to assess driving proficiency given by a licensed first line leader, NCO, or License
Instructor / License Examiner. The purposes of check rides are to assess driving
proficiency, address topics of importance prescribed by the Commander, and identify
weaknesses. Completion of sustainment training will be annotated on DA Form 348,
Section III or in the remarks area of DA Form 5983-E. Road test Form DA Form 6125
will be used and annotated on the top of form as check ride.
6-2 INDIVIDUAL FORMS An individual's DA Form 5983-E and DA Form 348 will be
reviewed annually for:
a. Safety Awards/Accident-free Driving Awards.
b. Expiration of Permits.
c. Accidents and moving traffic violations.
d. Remedial, required, or refresher training.
e. License suspension/revocation.
f. The review will be conducted and signed by the unit Commander or appointed
representative.
6-3 REMEDIAL TRAINING
a. Drivers or operators who have had a driver at-fault accident or traffic violation,
misused equipment, or have otherwise demonstrated a need for additional training on
military vehicles or equipment.
b. This training will be directed towards identifying and correcting individual
weaknesses and not as a form of punishment.
c. Remedial Training will be documented on DA Form 348-E, section III.

CHAPTER 7
SPECIALIZED TRAINING
7-1. SPECIAL DRIVER’S TRAINING. Driver training is required for Soldiers prior to
operating army vehicles OCONUS in order to meet host nation and MACOM
requirements. All host nation requirements must be researched prior to deployment.
Inquiries to the DOT sponsoring unit about licensing requirements are an essential part
of the pre-deployment conference/coordination. Training will include in-country driving
regulations, safe driving practices, and identification of international road signs.
Completion of this special training must be annotated on the Operators Permit ID/DA
Form 5984-E and entered on the DA Form 348/DA Form 5983-E.
7-2. FORKLIFT DRIVER’S TRAINING. Special operating instructions for forklift
operators can be found in the -10 operator manual. Further information can be gained
by going to TC 21-305-20, Ch. 20. This pertains to basic safety operations of the forklift
under any conditions.
7-3 CONSTRUCTION EQUIPMENT Applicants are required to be tested as specified
in TB 600-2 for graders, wheeled and track type tractors, front loaders, motorized
scrapers, rollers, sweepers, crawlers, track and wheel mounted cranes, mixing plants,
rock crushers, HYEX, snow plows, ditching machines, and saw mills.
7-4. HAZMAT TRAINING: Military vehicle operators must meet all licensing
requirements according to AR 600-55 and this SOP. This training must, as a minimum,
be an introduction or familiarization training per Part 171 Title 49 Code of Federal
Regulations. This training can be completed by completing the Defense Ammunition
Center’s, “AMMO-67 Course”, “Hazmat Familiarization and Safety in Transportation”.
Hazmat training must include but is not limited to the following:
a. Definitions of Hazardous Materials.
b. Placard Requirements.
c. Handling (loading and unloading) of hazardous materials.
d. Regulations and procedures for transporting hazardous materials.
e. Operations of emergency and spill equipment.
f. Forms and records including accident reporting.
g. Blocking and Bracing.
h. Vehicle Parking.

7-5. NIGHT VISION DEVICES: (NVD) Commanders will determine what type of night
training program best supports the unit mission and understand the night vision device
limitations in order to conduct effective risk assessment before NVD use (See TC-21305-20 and TC 21-305-2 for information on NVDs to assist in these decisions and
assessments.).
a. Instructors for NVD training/testing must be NVD qualified (i.e. completed the
"Training Program for Night Vision Goggle Driving Operations", TC 21-305-2), be
licensed on the vehicle on which training is conducted, be a SGT or above, and
appointed by the Commander as and NVD certified instructor.
b. NVD Qualification Training will include sequential instruction in both academic
and night driving tasks. A qualified NVD instructor must supervise all instruction and an
NVD equipped instructor will be in the vehicle during night driver training.
c. The minimum required NVD academic subjects are listed in Appendix I of AR
600-55.
d. A written test (See TC 21-305-2) must be passed for NVD qualification.
e. NVD performance tasks are conducted sequentially in five phases under the
supervision of a qualified NVD instructor. A summary of these tasks is shown below
(See AR 600-55 Appendix I for all tasks within each phase).

Phase I Tasks: Drive the vehicle without the NVD over a known range
during daylight conditions; become comfortable with the actual road and terrain.
(2)
Phase II Tasks: Using headlights drive vehicle without the NVD over the
same range at night.
(3)
Phase III Tasks: NVD system checks, fitting and use during PMCS.
(4)
Phase IV Tasks: With all external and internal lights off, drive vehicle over
the same range while wearing the NVD.
(5)
Phase V Tasks: Post operations check and AAR.
f. To be qualified and licensed on NVDs, motor vehicle operators must be licensed
and current on the vehicle in which they will receive NVD training.
g. When documenting the applicant's DA Form 348/DA Form 5983-E and
Operators Permit ID/DA Form 5984-E, instructors will identify the specific type of NVD
on which the individual is qualified.
h. Refresher NVD Training. Refresher training will be conducted for motor vehicle
operators who have not participated in an NVD driving mission in the past 12 months to
maintain or regain proficiency. Minimum requirements for NVD refresher training are:

7-6 RECOVERY TRAINING
a. Commanders will take action to ensure that individuals in the maintenance arena
assigned to Recovery attend school and upon completion receive the “H8” Additional
Skill Identifier (ASI) qualification on their licenses for Wrecker, Winch, Cable Operated
Cranes, and Tow Bar certification as applicable.
b. This information can be obtained by reviewing the MTOE and reviewing the
remarks portion to identify who has to have the Additional Skill Identifier (ASI) of H8.
c. Support activities will ensure that their personnel receive recovery training and at
a minimum will be Wrecker Qualified.

EQUIPMENT REQUIRING LICENSING FOR OPERATION
D-1 LICENSING REQUIREMENTS: All military, civilian, and DOD contract personnel
must have an Operators Permit ID/DA FORM 5984-E and demonstrate their proficiency in
order to operate the following mechanical or ground support equipment that is either
owned or leased by the Government:
a. Electrical power generating equipment, 0.5KW and above (electric motor driven,
diesel engine driven, gasoline engine driven, and gas turbine driven sets).
b. Gas generating equipment, all sizes and capacities (such as oxygen, nitrogen, and
acetylene).
c. Water purification sets, all sizes and capacities except those issued to individual
Soldiers for their personal use.
d. Air compressors, all pressures (Not to include installed automatically controlled
units.).
e. Material or cargo handling equipment, all sizes and capacities, for example, forklift
trucks, warehouse tractors and cranes, straddle trucks, and flight line tugs.
f. Bridging equipment, all bridge erection boats, mobile assault float bridge or ferry
transporters, and outboard motors.
g. Construction equipment or off-road equipment, all sizes and capacities, including
but not limited to: tractors, wheeled or tracked cranes, wheeled or tracked front loaders,
small emplacement excavators (SEEs) motorized or self-propelled scrapers, rollers,
sweepers, and earth augers, saw mills, chain saws, snow plow-rotary, ditching machines,
rock crushing and screening plants, asphalt and concrete spreaders, water and
bituminous distributors, hot oil heaters, and hydraulic or mechanical vibrating tampers.
h. Heating and cooling equipment, all sizes and capacities (Such as air conditioning
and refrigeration units powered by liquid fuel engines, space duct-type heaters using
liquid fuel, and steam cleaning equipment.).
i. Pumping equipment; all pumps 50 gallons per minute (GPM) and above, when
powered by liquid fuel engines.

DOCUMENTATION AND TRAINING FOLDER.
F-1 Training Documentation: In accordance with AR 600-55 all training will be
documented using DA Form 348 and DA Form 348-1-R. In addition, the following
created forms will be used to ensure all training requirements have been successfully
completed and documented. All forms will be compiled into the individual’s Drivers
Training Folder (tri-fold file folder, NSN 7530-00-707-8406). Supporting documents
(training/tests) will be placed in the individuals training files and will not be destroyed.
F-2 Disposition of Documentation: The required training documentation as listed
above will be maintained following the below listed guidance:
a. Copy of Civilian License: (Required) A copy of the current valid civilian driver’s
license will be maintained in the training folders. (Must be changed each time the
individual renews civilian license.)
b. DA Form 348 and DA Form 348-1-R (Required) Documentation must be filled
out with all drivers’ qualifications and signed by Commander’s authorized
representative.
c. Commanders Interviews: (Required) Commanders interviews are required prior
to that commander issuing the driver’s license. A commander’s interview for the
commander’s signature on the current license issued will be maintained in the training
folder.
d. Hazmat Tests: (Required) Copies of the Hazmat written test scores will be
maintained until replaced with the re-certification test every 2 years.
e. NVD Finial Written Test: (Required if Applicable) NVD written test is only
required for initially qualifying the individual on the NVD’s. NVD’s must be re-certified
every two years but only requires the Driving Road Test to be completed. The Initial
written test will be transferred onto the DA Form 348 and maintained in the training
folder for three years. After the initial 3 years and transference to the DA Form 348 the
written test for the NVD’s may be discarded.
f. NVD Road Performance Test: (Required if Applicable) The NVD road
performance test will be maintained in the folder until replaced with the new
performance test every 12 months.
g. DA Form 6125 Initial Road Test: (Required) The DA Form 6125 road
performance test will be maintained as well as follow-on Bi-Annual Sustainment Road
tests updated every two years.
h. Bi-Annual Sustainment Road Test 6125: (Required) Each Bi-Annual
Sustainment Road Test will be documented using the DA Form 6125. This form will
replace the initial road Test and then be replaced bi-annually with each Bi-annual
Sustainment completed.
i. Drivers Training Folder Check Sheet: (Suggested) the enclosed training folder
check sheet Should be maintained inside the folders, re-done, and replaced annually as
part of the annual review process.

j. Training Verification (via previously issued license): (Suggested) If this is from
DA Form 348 the 348 will be permanently left in the training folder. If not from the DA
Form 348 and not listed above, then once transposed onto DA Form 348 and DA Form
348-1R may be discarded if older than 3 years.
k. Equipment-Specific Training Form: (Suggested) The equipment-specific Training
Form is completed for each item trained and tested on after the initial training and
licensing. This Form is used to document training conducted and is transposed onto the
DA Form 348 and DA Form 348-1-R as qualifications and training remarks. Replace
and destroy old form once updated form is completed and filed.
These files will be constructed in the following manner:
a. Left File: Copy of current GCSS-A and manual DA Form 348 and state
issued driver’s license.
b. Center File: Two-Year Renewal Notice and current training
documentation.
c. Right File: All historical operator personnel qualification records.
NOTE: Unit clerk will notify the Clerk/Motor SGT of personnel changes, so that
individual training folders can be transferred to the gaining unit. Inactive training
records will not be destroyed. Inactive training records will be placed in the
inactive file.

TAB A: INTRODUCTION
1.
This guide is intended for use by the Unit’s Master Driver and Commander. The
guide’s objective is to improve the safety and training of our operators and ensure
compliance with regulatory requirements (AR 600-55, dated 17 September 2019, and
372nd QM BN (PS) Drivers Licensing and Dispatching SOP). Units may have existing
programs suitable for your unit; however, it is the process and standards which this
guide is designed to illustrate and ensure we are licensing to standard.
2.
Focus on the "licensing steps"- these will be the keys to a good training program.
They are intended to clarify and reflect the regulatory requirements of Army Regulation
600-55 and provide a basic structure for units to build upon with their own ideas.
Commanders who want to use a different program than described in this guide must
ensure they are in compliance with AR 600-55. Other tabs of the guide are additional
references.
3.
Use documentation individual checklist to ensure that all necessary supporting
documents are included in the Individual Licensing Packet. This form may serve as a
cover page for each operator’s Individual Licensing Record.
4.
Strongly encourage all individuals to retain a personal copy of all documents in
their licensing record in case the unit copy is lost or destroyed.

TASK:
Verify the applicant’s civilian driver's license. Copy individual's State
driver's license and one copy of a previously issued DA FORM 5984-E, Operator Permit
ID (if applicable). Retain copies with the individual's driver training packet.
CONDITION:
Copy civilian state driver license and any previous DA FORM 5984-Es,
Operator Permit ID (if applicable).
STANDARD:
Determine that the state license has not expired, document any
restrictions such as eyeglasses, financial responsibility, etc. Ensure that the DA Form
5984-E expiration date does not exceed 5 years or the expiration of the individual’s state
driver's license.
REMARKS:
If there is any reasonable doubt regarding the authenticity of a Soldier’s
state driver’s license, unit Master Driver must validate the license.
COMMANDER’S INTERVIEW

TASK:
When a commander or his representative select personnel to be drivers, an
interview must be used for the selection process.
CONDITION:
The Commander's Interview will be recorded and placed in the
individual’s drivers training folder and appropriate annotations made on the DA Form 348.
STANDARD:
Commanders are to interview subjectively to determine the driver's
maturity, attitude, past driving record, nervousness, or any abnormal characteristics.
REMARKS:

interview to be delegated to an E-6 or above.

TASK:
Ensure the applicant possesses the minimum physical ability to safely
operate the military equipment/vehicle for which the license is being sought. (372nd
QM BN (PS) Soldiers must produce a current and valid State driver's license to obtain
an ARMY STANDARD operator permit. The instructor must determine if the potential
operator has the physical ability and coordination to operate the equipment in a safe
and prudent manner).
CONDITION:
Current State license and the equipment on which to be licensed.
STANDARD:
Observe the potential operator throughout the specific training and
testing process to ensure he or she has the physical attributes needed to operate the
equipment. The Drivers Testing and Training Device can be used for this process.

REMARKS:
Physical evaluation tests are intended for diagnostics, guidance, and
counseling purpose(s). They will ensure that all operators of motor vehicles possess
the physical requirements for safe driving.

TASK:
Instruct the license applicant on the safe operation and characteristics
of a piece of equipment or vehicle. Instruct license applicant on the introduction to the

CONDITION:
Given appropriate manuals, training aids, and a qualified License
Instructor with adequate knowledge about its characteristics and operation.
STANDARD:
The Soldier applying for the license demonstrates an understanding
of the safety procedures and limitations. The applicant must also display ability to
perform Risk Management and accident avoidance techniques.
REMARKS:
Safety is a command responsibility. Properly and continually
emphasized, safety awareness will prevent needless manpower and equipment loss
and contribute toward the readiness of our units.
The potential operator for a vehicle/equipment license must receive detailed instruction
in the safe operation of the equipment. The safety training must emphasize measures to
prevent commonly occurring accidents and avoid hazards related to the operation of the
equipment.
The licensed applicant will be instructed in operator responsibility, the role "attitude"
plays in safe operation, and liability of military equipment.
Phase II training will be conducted for each specific vehicle or equipment type that is the
operator is to be licensed.

TASK:
The Learner Permit is issued to a license applicant after completion
of licensing steps 1-7 and prior to operating the equipment. The Army Learner Permit
must be issued before conducting hands on training and the Phase III operation/road
performance test.
CONDITION:
Operator must have received Phase I and II training and have
passed Phase I and II written examinations. Given instructions – DA Form 5984-E
Government Motor Vehicle Operator's Identification Card, stamped "Army Learner", is
valid for the operation of military vehicles within specific limitations prescribed in AR
600-55, Para 6-3b.
STANDARD:
Issue Army learner permit after completing appropriate classroom
instruction and examinations, but prior to Phase II hands on training. The learner permit
will be surrendered upon completion of training.
REMARKS:
After the Learner Permit is issued the License Instructor must
conduct/coordinate the training to prepare the applicant for safe operation and
performance testing.
The issuance of a learner’s permit is a necessary requirement for legal reasons before
operating a vehicle on public roads. This requirement does not apply to ground
equipment (i.e. generators, pressure washers, welding equipment, etc.).

controls, features, capabilities, limitations, and safe operation of the vehicle/equipment
being trained.
REMARKS:
Comprehensive and detailed training is key to having safe, skilled,
and knowledgeable operators. Properly trained operators should have a complete
understanding of their equipment. For example, an LMTV driver should be able to
understand and explain to the License Instructor such details as how the Central Tire
Inflation System (CTIS) works and how it is used, and how the Warmup/Retard switch is
used with the engine exhaust brake system.

TASK:
The unit licensing examiner will administer the road test and
complete the road test checklist IAW DA Form 6125.
CONDITION:
Given appropriate vehicle/equipment operator's manual, DA Form
2404/5988-E, DA Form 6125, and a designated driving test course which meets the
requirements of the DA Form 6125, and qualified License Instructor/Examiner.
STANDARD:
The road test will consist of 3 scored phases. PMCS test, the
vehicle control test, and the on-the-road test.
** If the applicant fails any phase of the test, the entire road test will be terminated
at that point.
REMARKS:
The unit License Examiner will establish a standard driver test
course, which includes the required tasks from AR 600-55, Appendix G.
The purpose of the road test is to evaluate the driver's ability to drive safely in most onthe-road situations. It serves as a basis for issuing an operator's permit and provides for
instructional reinforcement and counseling.

TASK:
Conduct and document annual records review and two-year
refresher driving test.
CONDITION:
Given the Operators individual drivers training folder and supporting
documentation, DA Form 6125, Road Test Score Sheet and DA Form 348.
STANDARD:
First line supervisors, who are qualified and licensed to operate the
equipment/vehicle, will conduct two-year check rides for each driver (ARNG). The twoyear refresher driving test will assess driver proficiency, identify weaknesses, and help
improve unit driving training program. Completion of sustainment training will be
annotated on DA Form 348.
REMARKS:
Every operator's individual Licensing/Driving record will be
reviewed annually by the Unit Licensing Examiner. The review will identify:
Safety and driving awards earned
Expiration of permits
Accidents, violations, or license suspension (civil or mil)
Sustainment training is periodic driver training conducted to maintain a high level of
driver skill proficiency and to prevent drivers from acquiring poor habits. This road test
will be completed in a vehicle endorsed on the Soldier’s license.
Add the DA Form 6125 recording the 2-year refresher driving test to the Soldier’s
licensing file.

TASK:
Conduct training and licensing documentation for night driving
using a night vision device (NVD). Drive vehicle during night conditions.
CONDITION:
Given instruction, DA Form 5988-E/2404, DD Form 1970/5987-E,
pencil, appropriate vehicle operator's manual, equipment records folder, rags,
lubricants, coolant, improved road surfaces, appropriate night vision device (NVD) and
wheeled vehicle with BII.
STANDARD:
Without accident or injury, drive the designated route at night while
employing vehicle black-out drive capability; use defensive driving methods. Up-shift
and downshift the transmission, manipulate the controls, use correct braking
procedures, and perform basic driving maneuvers.
