Safe Work Procedure NREN M 024 SWP-76 Revision Number: 1 Date: 07.March.2012 BOAR QDS Drill Rig Set up and Operation 1. PURPOSE The purpose of this Safe Working Procedure is to document a safe method of working whilst setting up and operating a BOAR QDS Drill Rig in the underground sections of the mine. 2. SCOPE This document applies to all Mine Personnel and contractors that are suitably trained and appointed to operate the BOAR Drill Rig. 3. REFERENCES This document is to be read in conjunction with relevant sections of: • MSMP 004 Major Hazard Management Plan Transport • MSMP 011 Isolation Management Plan • MSMP 022 PITS Scheme • NRE 002 Risk Management • Managers Support Rules for the area/panel. • NREN T 079b ALFABS QDS BOAR Bolter Training Package. 4. MACHINE SPECIFIC HAZARDS o Release of Energy ( Mechanical, Hydraulic Pressure, Gravity, Water pressure) o Rotating parts o Unintended activations if Rig not correctly hosed up ie must have return line directly connected to tank o Unintended activations/movements of the BOAR if LHD not placed in bolt mode. o Exposure to Roof and/or Rib sprawling o Personnel Interaction with Mobile Machinery o Manual Handling of gear into BOAR and during bolting o Slip, Trip, Fall due to poor Housekeeping o Pinch Points around drill mast in front of operators console o Chemical exposure o Dust, Fumes, Noise 5. GUIDELINES Only personnel trained (or in training) in the safe operation of the BOAR QDS Drill Rig shall operate the equipment. Such persons, as mentioned above, must be trained and assessed as competent as per the mines Training and Competency Management Plan (MSMP 028) and authorised by the mine manager. Note 1: SITE INSPECTION a) Deputy to complete statutory inspection. b) Work Crew to identify work area and identify hazards, bar down loose material and conduct house keeping to ensure a safe work site Note 2: Pre Shift Communication AT COMMENCEMENT OF SHIFT a) Supervisor / Deputy to Communicate with all relevant parties the location and expected duration of the bolting process. Note 3: ASSEMBLE EQUIPMENT, MACHINE AND TRANSPORT TO WORK SITE a) Appointed operators to conduct PITS inspection on LHD and BOAR Drill Rig b) Assemble all supplies needed for the bolting process in the designated baskets on the BOAR QDS Drill Rig. c) Ensure all equipment is fit for purpose and has been inspected prior to being used. Note 4: MANUAL HANDLING a) Two person lift at times when needed to avoid any strain injuries. b) Avoid lifting and twisting at same time c) Use More frequent lighter loads Work Method: 1. SET UP JOB SITE a) Ensure Statutory supervisor has inspected the work area. c) Crew to conduct as a minimum a SAM or JSEA if required or if task is unfamiliar or involves inexperienced personnel. b) Require water supply to Rig. No ball valves on flexible hoses and secure with Safety clips c) No machine movements while pedestrians within 12 meters of LHD on man riding in QDS Bolting Rig 2. CONNECTION to LHD Connect up QDS Bolting Rig In accordance with MSMP-011 Isolation of Energy Management system. Check all connections are tight and correct. Once attachment is connected lift up the BOAR and visually check that tongue is fully engaged. Failure to do this may result in back pressure in the return line causing Unintended Activations of the rig Connect pressure line to QDS pressure coupling and case drain to the LHD Case drain coupling Connect the Air Safety circuit to the LHD. Remove/Clean up any build up of material in the QDS Bolter 2. Check the 3 BOAR hydraulic E/Stops are functioning. 3. Ensure the 2 LHD E/Stops are functional. 4. Visually inspect the flooring for any damage ie deformation 5. Visually inspect handrails for damage 6. Test the operation of the Doors – Self Closing and Self Latching Complete 7. Visually inspect the Main structure for any damage Including obvious cracks in welds and bent or deformed structural members 8. Water hose’s were flushed before connecting 9. Ensure the safety ladder is in place and in good condition. 10. Inspect hydraulic hoses, gagues and couplings for damage and that they are routed back to the LHD where machine movement’s will not damage them. 11. Ensure QDS tongue is fully engaged 12. Check the hydraulic return line is connected directly to the hydraulic tank. Failure to do this can result in Unintended activations of rig components. 13. Inspect all guarding is in place. Note: If the above checks identify any defects they may prevent the safe use of the Bolting Rig, the Rig is to be placed Out-Of-Service, notify control and the appropriate Engineer or Mining Official so that repairs can be undertaken. This will disable the LHD steering and ensure the brakes are applied so the LHD operator cannot inadvertently move the BOAR while men are in it or bend the stab jacks.Raise the MIS/QDS lever to get hydraulics to the BOAR 5. 6. Ensure load does not exceed 2 Men plus 500kgs 7. Position BOAR in bolting location 8. BOAR operators to mount while rig is on the floor 9. Test all functions 10. Engage the water trickle valve if LHD oil overheating could be a problem 11. Must have LHD operator at all times the BOAR is in use 12. Set up hand rail(s) if required 13. BOAR operators to clearly communicate to LHD operator when ready to raise the BOAR 14. Lower the stab jacks 15. Ensure no rig operations while any offside is in front of control console. Operators are to activate the E/Stop before entering this area. 16. Commence drilling and bolting as per training 17. When drilling completed: a. Retract drill rig b. Slew rig to LHD drivers side to give best vision during transport c. Raise floor jacks d. Lower BOAR e. Men out of BOAR while on the ground 18. Men are not to be transported in the BOAR other than for minor repositioning. Store the BOAR in designated storage location where it should not be inadvertently contacted by passing vehicles No Standing Zones Access to the area in front of the BOAR operators console by the offsider can only be done when the rig is fully positioned and with permission from the operator and with the E/Stop activated While Bolter is connected to an LHD, no standing zones of two metres are to be followed unless the LHD is turned off and Bolter is resting on the ground. Note: SWP’s are designed to document a clear safe process for undertaking a task and/or activity. As such they must include outcomes from a specific risk assessment.
