AL SOLAI SAFETY MOMENT WHAT HAPPENED…. !!! On 23rd January 2019 around 12.30 am, one of our loader operators was feeding crusher machine along with one helper. but suddenly their loader stopped working due to some electrical fault. Instead of waiting for the mechanic to come and repair the loader they decided to fix it themselves. The helper asked the operator to sit in the cabin and try to start the loader while he was sitting on the back of the tyre and attached cables to its battery. As the operator tried to start the engine, suddenly it moved forward with a jerk and unfortunately the helper sitting on the tyre got stucked between the tyre and mudguard and died on the spot. His body stucked there for almost 6 hours and later on was shifted to the hospital. WHY IT HAPPENED…? 1. No supervision…... As there were limited number of workers working, so the management decided not to deploy any safety officer. 1. Work pressure…... There was immense pressure on operator to finish the task at any cost that forced the operator to take short cut. 1. Personal negligence… Instead of waiting for the mechanic to repair the loader, they tried to fix it themselves. LESSON LEARNED…... 1. No matter how small the activity is, it must be supervised all the time. 2. Short cuts always lead to accidents. 3. Achieving work targets is important but not cost of someone’s life. at the
