For this school year, I am teaching Media and Information Literacy, Practical Research 2, and General
Mathematics. I am also designated as the Senior High School eBEIS and LIS in charge. As a Senior High School
teacher in Ozamiz City School of Arts & Trades, here are the following things I have accomplished for the month of
July of this school year, to wit:
a) Submitted the log sheets weekly for checking
b) Submitted the accomplishment report June 2018
c) Prepared Form 4 for the month of June 2018
d) Assisted my co-teacher in Subayan Keg Subanen Festival as a member of the committee in charge in water
distribution
e) Completed and submitted the template for the DepEd email (SHS)
f) Attended the Teachers Work Immersion
g) Computed the KPI of the SHS for the SY 2017 – 2018
h) Attended the meeting for the implementation of the OK Oplan Kalusugan
i) Filled up the SF8 and submitted it to the school nurse
j) Submitted the number of 4P’s beneficiaries in the SHS
k) Submitted the list of IPs in the SHS to Mrs. Navales
l) Submitted the compiled SF 1 and SF2, and the filled up SF4 to the office of Mrs. Magpulong
m) Submitted my accomplishment report for June 2018
n) Created classes in the LIS
o) Enrolled students in the LIS
p) Resolve issues encountered in enrolling students in the LIS
q) Facilitated in the feeding and deworming of my Gr 11 B2

eBEIS and LIS in charge and was assigned as the adviser Grade 11 Block 2. As a Senior High School teacher in
Ozamiz City School of Arts & Trades, here are the following things I have accomplished for the month of June of this
school year, to wit:
a) Attended the school year’s opening program
b) Oriented and implemented the school rules and regulation.
c) Disclosed the topics for the subjects Media and Information and Practical Research 2
d) Acquainted the students on the grading system and its components
e) Elected classroom officers
f)
Filled forms for Senior High School Monitoring Template and Tracking Template
g) Summarized enrollment for the quick count
h) Checked and updated the DepEd emails of the SHS personnel
i) Submitted the User Data for the DepEd email for SHS teachers
j) Presided the HRPTA Meeting and elected the HRPTA Officers last June 14, 2018
k) Submitted the DLL with curriculum guides
l) Attended the symposium facilitated by LTO and PNP
m) Prepared the SF 1 and SF 2 of my advisory class
n) Attended the SHS Conference at the Division Conference Hall last June 22, 2018
o) Facilitated the profiling of the students’ reading skills
p) Facilitated in the weighing and measuring of the height and weight of my advisory class last June 26, 2018
