Supplemental:
The IEP team has determined that supplemental aids and services are not needed in the general education or special
education setting for Anna to be successful in her least restrictive environment.
Modifications:
The following modifications are needed in all educational environments to aid in her documents learning needs related to
reading and math. The teacher providing instruction is responsible for these modifications with the support of her special
education teacher.
- Anna will be provided preferential seating in her general education classroom to help ensure that she is attending
during instruction and work-time.

- Anna will be provided frequent check ins to ensure understanding for content being covered in class.
- Anna will be provided additional time for assignments/tests assignment as seen appropriate my general education and
special education staff.
- Anna will have tests read aloud that are not measuring reading skills.
Assistive:
The IEP team has determined that assistive technology is not needed in the general education or special education setting
for Anna to be successful in her least restrictive environment.

Goal #1
Area: Social, Emotional, Behavioral
Goal:
By May 2021, Anna will increase her reading fluency and comprehension FROM reading 82 words correct in one
minute with 96% accuracy on a cold read at a level 4.5 reading level and comprehending the passages with 80%
accuracy TO reading 90 words correct in one minute with 96% accuracy on level 5.5 reading passages and
comprehending the passages with 80% or better as measured by teacher checklists and documented by special
education staff.

Area: Academic Performance
Goal:
Anna will increase her math functional skills FROM being able to understand and solve math problems using whole
numbers, estimation, factors, fractions, geometry and measurement, data and statistics, and problem solving with 48%
accuracy, to being able to complete these skills with 80% accuracy as measured by his special education teacher.
