PART I - ADMINISTRATIVE DATA Rank/Grade Name (Last, First, MI) BEARD, DANIEL SPC/ E-4 Organization Date of Counseling 10APRIL2018 Name and Title of Counselor D CO. 2-502 IN, 2ND BCT, 101ST ABN (AIR ASSAULT) SSG MEJIA, DAVID SECTION LEADER PART II - BACKGROUND INFORMATION Purpose of Counseling: (Leader states the reason for the counseling, e.g. Performance/Professional or Event-Oriented counseling, and includes the leader's facts and observations prior to the counseling.) - Initial Counseling: SQUAD LEADER PART III - SUMMARY OF COUNSELING Complete this section during or immediately subsequent to counseling. Key Points of Discussion: This is your initial counseling. 1.) TIMELINESS: Make sure you are present at least 15 minutes prior to all announced formations. 2.) ACCOUNTABILITY: Be where you need to be, when you need to be there, with the proper gear and or the right uniform. In all events, you need to keep me, or in my absence one of the other NCOs in our platoon aware of your whereabouts at all times. 3.) UNIFORM AND APPEARENCE: Your uniform needs to be presentable at all times. there is no such thing as a "field uniform". However you do not have to wear your best uniform to work in the motor pool. At no time should your uniform fall below the standards set forth in AR 670-1. Also, you need to make sure you are in the correct uniform for the mission. If you are unsure of the correct uniform and I have not given you clear direction do not be afraid to ask. You are required to be clean shaved at all times (on duty). You will have a fresh hair cut at least every two weeks. 4.) DUTY: If you have duty, you need to ensure that you are in the proper place, at the proper time, and in the correct uniform for that duty. I will inform you of any duties you may have as soon as I am made aware of them. Be aware that things change at a moment's notice, so be prepared in case that situation arises. Always be prepared and plan ahead. 5.) ATTITUDE: You need to have the proper attitude at all times. Show courtesy and respect to leaders, peers, and subordinates. Always be professional and never allow your personal feelings to get in the way of respecting other Soldiers. Stay motivated in the worst of times. One attitude can affect your whole team. Always help your peers out if they are in lack of motivation; they will do the same for you. 6.) APPOINTMENTS: If you have an appointment, let me know the time and purpose of the appointment as soon as possible. Preferably, directly after you make the appointment. If you need to make an appointment, check with me before making the appointment so I can check the training schedule.. This includes any appointments that your spouse/family member may need that will cause you to stay at home or take care of your child. Or in situations that require transportation. 7.) PERSONAL ISSUES: Any personal or family issues that arise, make sure to inform me ASAP so that I can give you whatever support you may need. Do not be afraid to approach me with any concerns you may have. I cannot help a problem if I don't know the problem exists. If I am not available do not be afraid to jump the chain of command. But first and foremost utilize your change of command. 8.) PT: You will exceed the army standard. The AIR ASSAULT Big 5 requires Soldiers to uphold a 270 or higher. You need to consistently prepare yourself to achieve this goal every time. I recommend you shoot for a 300. Remember, if you fail a PT test, you will be flagged and required to take a PT test every month until you have a passing one. Prepare yourself. 9.) COMMUNICATION: When I send out an e-mail, call or text message, you will send a reply as soon as you receive the message. I understand you can't always have your phone or computer with you, but when ever you do read it, send the reply, IOT keep the information flowing smoothly up the chain of command. 10.) PROFESSIONALISM: As a Soldier you are required to think. You volunteered to be here. You are a professional. This means that you will always look to improve not only in physical fitness but with your weapon systems and your equipment. Your courtesy to superior officers will set the example throughout the battalion. Failure to improve or to act in a professional manner will cause you to be recommend for unfavorable action. Always fight for your spot in the team. Constantly learn from others around you. Learn how to use your resources. We are one team; work together and have an open mind. "Know who you are, know what you represent, and know what you come back to." Other situations may arise that are not covered in this counseling session. If you are unsure about anything, contact me and I will help you understand. It is my duty as an NCO to help you develop into a better Soldier. However, I cannot do this alone. You must put forth the effort to make yourself a better Soldier, and in return I will help guide you in the right direction for your military career. _____ I fully understand the expectations set forth in this counseling statement and choose to uphold the values set forth to the best of my ability.
