Bank Account Statement: Must have been issued in the last 12 months and show your current Minnesota address. Residential Lease Agreement: Must have been issued in the last 12 months and show your current Minnesota address. Out of State Driver License/Permit You will be asked to verify that the information you provided in your online pre-application is correct. This confirmation page does not guarantee a Driver's License/ID. Additional documents may be required to complete your application. If you do not appear at a state driver services office to complete the application you will NOT receive a Driver's License/ID. Before visiting your local office, please check their driver’s license application hours online at dvs.dps.mn.gov under the Locations tab. Many offices have different hours for each type of driver’s license application (standard, REAL ID, enhanced). Checking before your visit will help make sure you have time to submit your application.
