The Minnesota Department of Public Safety Driver and Vehicle Services Division (DPS-DVS) has received
your pre-application for a Class D driver's license (REAL ID-compliant). Please bring this confirmation code
and other required documents noted below to any Minnesota Driver’s License office to complete your
application within 30 days of this letter. You must provide original documents or documents certified by the
agency that issued them. A credential cannot be issued without these proof documents. Digital documents
shown on your phone or other electronic device will not be accepted. If you are under the age of 18, a parent or
legal guardian must be present.

Required Documents:
Social Security Card: Must be original, not a photo copy. May not be laminated or otherwise altered.
Metallic Social Security Cards will not be accepted.
US Passport Book/Card: This document cannot be expired.
Bank Account Statement: Must have been issued in the last 12 months and show your current Minnesota
Residential Lease Agreement: Must have been issued in the last 12 months and show your current
Minnesota address.
Out of State Driver License/Permit
You will be asked to verify that the information you provided in your online pre-application is correct.
This confirmation page does not guarantee a Driver's License/ID. Additional documents may be required to
complete your application. If you do not appear at a state driver services office to complete the application you
will NOT receive a Driver's License/ID.
Before visiting your local office, please check their driver’s license application hours online at dvs.dps.mn.gov
under the Locations tab. Many offices have different hours for each type of driver’s license application
(standard, REAL ID, enhanced). Checking before your visit will help make sure you have time to submit your
