anonymously. Please score each of the cultural elements from 0 (Poor) to 3 (Ideal) in the
score column. Match the description with what you perceive the culture is in your
company. It is important that you are honest in your answers. The results of the
assessment are tabulated and evaluated by Texas Mutualsm and suggested improvement
activities will be submitted to the company in an effort to create a Total Safety Culture.
