PROCEDURE:
1. Complete the documents required. Put all the documents in 1 large brown
envelope (with the applicant´s surname, name and contact Lumber written in bold
letters on the upper left corner of the envelope).
2. The applicant must come personally on the appointment date and must pay the
visa fee upon submission of the application.
3. Always include 2 photocopies of ítems no.3, 4, 5, 6 & 7. Three (3) photocopies of
item 1.

1. Duly accomplished National Visa Application form.
2. One (1) recent passport-size photo with white background (to be pasted on the
application form)
3. Valid passport (with validity of at least 12 months).
4. 1 photocopy of the data page of the valid passport.
5. Duly accomplished application Form EX–01.
6. Duly accomplished Form 790.
7. Proof of economic means: bank certifications, financial statements, etc. With a
minimum amount of 2,151.36 euros monthly income, and an additional amount
of 537.84 euros per month for every dependent family member.
8. Medical/Health Insurance by insurance company accredited in Spain.
9. Letter written in Spanish from the applicant explaining the motive for the
application for the visa and indicating the place of residence in Spain.
10. Medical Certificate (done at any DOH accredited hospital or clinic) indicating the
non-existence of diseases that may have a serious effect on public health, according to
International Sanitary Regulations of 2005 (authenticated by the Department of Foreign
Affairs) (validity 3 months from date of issuance).

11. NBI Clearance (National Bureau of Investigation Clearance) validity must be
6 months from the date of issuance, duly authenticated by the Department of
Foreign Affairs; as well as Police/NBI Clearance from the places of residence within
the last 5 years, authenticated by the authorities of that country and the
corresponding Spanish Consulate/Embassy.
12. All documents presented must be translated by a recognized Spanish translator
(traductor jurado) accredited by the Spanish Ministry of foreign Affairs.
NOTE: All Filipino documents to be used in Spain in applying for the residence
card have to be authenticated by the Consulate General of Spain / OTHER
DOCUMENTS NOT INDICATED MAY BE REQUIRED

FOR SECURITY REASONS, CELLPHONES ARE NOT ALLOWED INSIDE THE
CONSULATE GENERAL. THEREFORE IT IS ADVISED NOT TO CARRY ANY
DURING YOUR VISIT, OTHERWISE YOU WILL BE OBLIGED TO LEAVE THEM
OUTSIDE THE CONSULATE GENERAL’S PREMISES. THE CONSULATE GENERAL
WILL NOT BE HELD LIABLE FOR ANY LOSS OF OR DAMAGE TO PERSONAL
