This Application Form can be photocopied/reproduced.
Only true and correct entries on this form are accepted; any false information will be a ground for your disqualification.
Write the code number in the testing center code box after selecting your preferred examination center from the table of Testing Centers.
Applicants have 2 options to file the Application Form:

Personal – From October (right after the last Entrance Examination) to September (the following year) @ PMMA Zambales only

Mail – From October (right after the last Entrance Examination) to August 31 (the following year) only
ADMISSIONS REQUIREMENTS:

3 copies of recent 2x2 colored pictures with name tag, front view; white background

2 self-addressed long white mailing envelopes with P40.00 worth of mailing stamp
per envelope for mailing of Test Permit and Examination Result.

Certification from School Principal that applicant is currently enrolled as Grade 12,
Photocopy of Senior High School Diploma or Form 138/Photocopy of Diploma for

EXAM:
The PMMA entrance examination covers the following subjects/areas: English (Reading Comprehension, Grammar, Vocabulary), High
school Mathematics (Algebra, Geometry, Statistics and Trigonometry), Science (Gen. Science, Chemistry & Physics), and Abstract Reasoning
Each applicant whose qualifications have been approved shall be given a permit to take the test by the Admissions Office.
For mailed applications, permit to take the test shall be sent to the applicant through mail. Please take note of the following:

Examination is from 8:00am – 12:00nn

Arrive one hour before the examination proper at your designated testing center

Bring your test permit (Show any valid I.D. if you did not receive your test permit)

Bring any valid Identification

Use only Mongol Pencil Number 2

Calculators are not allowed
SCREENING:
Successful examinees of the Entrance Examination will have to undergo and pass the following to qualify for admission:
7.1 Psychological Evaluation
7.2 Height/Weight, Color Perception Test (ISHIHARA), Snellens, Dental (Pre-Med/ Pre-Dental)
7.3 Physical Examination
7.4 Laboratory Examination
7.5 One-month Orientation Training (inside PMMA campus during the month of July)
QUALIFICATION REQUIREMENTS
Applicant should/must be:
 Male or female, physically fit, natural born Filipino citizen;
 Single and never been married and has no children out of
wedlock;
 Free from being accused or convicted of any crime against
the law and the state;
 At least 17 years old and not more than 23 years old upon
enrollment on August 01;
 At least Grade 12 student, College Level
 At least 160 centimeters (5 ft. & 3 in.) for male and 155
centimeters (5 ft. & 1 in.) for female;
 BMI of less than 25;
 At least good academic performance
 Has never been enrolled in PMMA.
GROUNDS FOR MEDICAL DISQUALIFICATION
 Defective Visual Perception (color blindness, near sighted/far sighted
with corrective eyeglasses or lens);
 Physical Deformities (bowlegged) congenital defects;
 Perforated ear drum, chronic sinusitis, bleeding tendencies,
hemorrhoids, hernia (if not treated), varicocoele, extensive skin
disorders;
 History of heart disease, hypertension, asthma, active PTB, kidney and
liver diseases, epileptic seizure disorders, major operations,
communicable diseases (such as AIDS, Hepatitis, VD, etc.);
 Full dentures, upper and lower; Cleft lip and palate, Malocclusion,
Deformities of the face, Open bite, Tongue-tied; Missing of four
anterior/posterior teeth.
 Tattoos and other brotherhood marks;
 Additional for Females: Goiter, History of pregnancy, presence of
breast mass, painful menstruation.

 Bachelor of Science in Marine Transportation
 Bachelor of Science in Marine Engineering
NOTE: In order for a Testing Center to be opened, a MINIMUM number of 100 applicants is needed. In case the number of applicants is less than 100, those applicants will then be
reassigned to another testing center nearest to the applicant’s preferred testing center.
